This paper, sample survey questionnaire, is for reference purposes only for those students who are having hard times in creating their survey questionnaire for their case study.
Abstract All empirical research involves some form of data collection. One of the approaches commonly used the human sciences, is survey research. This article focuses on the various forms of interviews and using the questionnaire technique as a data collection instrument often associated with surveys. It puts the different interview types on a continuum, ranging from structured to unstructured interviews into perspective against two underlying types of data, namely qualitative and quantitative data. The article sensitises the prospective researcher for some pitfalls when using the interview as a data collection technique and includes some hints for this protective researcher when using the interview data collection technique in practice. It also attempts to bring order into the vocabulary when using the concepts: procedure and technique.

Survey research is a non-experimental research approach used to gather information about the incidence and distribution of, and the relationships that exist between, variables in a predetermined population. Its uses include the gathering of data related to attitudes, behaviours and the incidence of events. Survey research in one form or another has existed for over two millennia with the population census of Caesar Augustus (St. Luke's Gospel) being an early example. For most modern researchers sample surveys are more cost effective and easier to undertake than population surveys when gathering information; however, this increases the risk of both representation and measurement errors. There are a number of different forms of survey research; however they all share common steps and common limitations. The purpose of this article is to discuss these steps with a view to highlighting some of the common difficulties.

In this chapter, we will first discuss the difference between concepts-by-intuition and the concepts-by-postulation. After that we will illustrate the different ways in which concepts-by-postulation can be defined by concepts-by-intuition. In doing so, we will make a distinction between concepts-by-postulation, namely between concepts with reflective and formative indicators. Questionnaire surveys dominate the methodological designs in educational and social studies, particularly in developing countries. It is conceived practically easy to adopt study protocols under the design, and off course most researchers would prefer using questionnaire survey studies designed in most simple ways. Such studies provide the most economical way of gathering information from representations giving data that apply to general populations. As such, the desire for cost management and redeeming time have caused many researchers to adapt, adopt and apply the designs, even where it doesn’t qualify for the study contexts. Consequently, they are confronted by managing consistent errors and mismatching study protocols and methodologies. This article focuses on the question of survey research in the social sciences. It is a unique method of data gathering in the social sciences, which involves the use of questionnaire and interview techniques. The questionnaire technique involve the use of instruments ranging from the loosely structured, highly structured, open-ended to closed-ended questionnaires that aim at collection of quantitative data. While interview technique includes the use of personal interviews, ranging from schedule-structured, informal, unstructured, to telephone interviews. This paper shows that that survey research is the main method for a social scientist with numerous strengths despite some weaknesses. In conclusion, the paper argues that survey research method can be adapted to the emerging advancements in science, information communication and technology.
