    DECLARATION AND ACCEPTANCE OF POLICIES

    I, _____________________________________, a student of the


    College of Nursing, Southwestern University PHINMA, _____ years of age
    and a resident of ______________________________________ after
    having been called in to a conference with the College of Nursing,
    Southwestern University this___________ Day of November 2022, do hereby
    depose and state that:

    I have been fully explained and completely understand the following


    important matters and/or facts related to my enrollment in the College of
    Nursing:

    A. the RAD Block Examination (RBE) is part of the grading system all
    professional courses/subjects in the College of Nursing. It is a requirement
    that all students must take and pass the RBE in order to pass the
    course/subject. In the event, the student fails the RBE during the 1 st take of
    the examination, he/she is given 1 opportunity to retake the said
    examination. However, should the student fail the RBE a second time,
    inclusive of the retake, the student shall be declared FAILED in the said
    course/subject for which he/she is unable to reach the minimum passing
    score for the RBE;

    B. the College of Nursing follows and implements the retention policy of


    Southwestern University PHINMA (the University). Students who have failed
    previous courses/subjects will be deloaded according to the said policy.
    Students who have failed in 4 courses/subjects throughout their enrollment
    in the College of Nursing will no longer be allowed to enroll in the succeeding
    semester;

    D. all BSN 4 students are required to attend the Enhancement Review


    program provided by the College of Nursing’s partner institution, TopRank
    Review Academy. Attendance to the said program is mandatory and is
    monitored by the College of Nursing. The review schedule is included in the
    RLE schedule. A student who is absent during review will automatically be
    sanctioned with a duty extension;

    C. in order to be eligible for promotion to the next year level and/or the next
    semester a student must: pass all subjects for which he/she is currently
    enrolled, complete all Related Learning Experience (RLE) extensions or
    repeat rotations, comply with all submissions and INC forms and paid all
    applicable fees;

    D. decorum and behavior is strictly observed. Students are required to wear


    the prescribed school uniform at all times during classes, RLE and while
    inside the campus. Students are expected to address/communicate with their
    teachers, staff and employees of the University at all times. Violation of
    policies on decorum and behavior are subject to corresponding sanctions.

    E. completion of the required Operating and Delivery Room cases is a


    requirement for graduation. BSN 4 students are required to complete 2 cases
    each for the following categories: OR Major Scrub Nurse, OR Major
    Circulating Nurse, DR Actual Delivery (Handle), DR Assisted Delivery (Assist)
    and DR Immediate Newborn Care.
