Risk factors should not exceed the Medium Risk level.
If they do, then control measures must be put in place to reduce the risk factor as per Stage 4.
(This does not mean that risk factors below the medium risk level would not need implementing).

Stage 3 Control Measures and Procedures to Reduce the Risk


This stage is to determine what measures would reduce the risk factor of a particular hazard identified
above.
These measures would either lower the severity of the hazard, or reduce the likelihood of it occurring, or
both. These measures can range from using additional safety gear, using backup systems, standardising the
procedures, regulating conditions under which the activity can be undertaken etc.

Care must be taken NOT to create new dangers by implementing control measures.

UNDERSTOOD by all

- The Enclosed Space must be thoroughly ventilated prior to entry

SUPERVISION BY OFFICERS

- Proper PPE and Equipment; ensure they are in working order and are maintenance on weekly
/ monthly basis as required
- An alternative means of communication may need to be established.
- Breaks should be taken at regular intervals;
- Drink sufficient fluids (water) to avoid dehydration - Consumption of alcohol prohibited, soft
drinks and coffee should be avoided;
- Suitable lifting equipment and chocks should be used when opening manholes, etc.;
- Consult “RADF G11 - Manual Handling' Risk Assessment”
- Prior to entry into Enclosed Space – Ensure sufficient cooling of Enclosed Space and ensure
person attending is medically fit
- All crew involved must be briefed and a Contingency Plan / Rescue Plan in place and
UNDERSTOOD by all
- Use of mechanical fans;
- Breaks should be taken at regular intervals;

- Drink sufficient fluids (water) to avoid dehydration - Consumption of alcohol prohibited, soft
drinks and coffee should be avoided;
- Where possible, heating of adjacent compartments (i.e. fuel tank) should be stopped.
- Equipment for Gas Detection and Respirator must be in good working condition / calibrated /
certified
- Emergency planning to be in place; Emergency Rescue Equipment: E.g. Tripods, Med First
Aid Kits to be readily available. Followed by supervision of Master and Officers
FCL 19-2018 PERSONNEL PROTECTIVE & RESCUE EQUIPMENT
- DRILL EXERCISE OF “RESCUE OF PERSON FROM ENCLOSED SPACE” – Enhance
knowledge / familiarization.
- Adequate lighting to be provided. Use intrinsically safe portable lighting, if required;
- Any opening in the deck (i.e. manhole, grating, etc.) should be secured with guard rails /
ropes and adequately lit.

- Permission must be obtained by the Master


- All crew involved must be briefed and a Contingency Plan / Rescue Plan in place and
UNDERSTOOD by all
- Proper PPE and Equipment; ensure they are in working order and are maintenance on weekly
/ monthly basis as required
- The Enclosed Space must be emptied (I.E. Ballast, Fresh Water Tank, ETC) Prior to Entry
- The Enclosed Space must be thoroughly ventilated prior to entry
- Ballast / De-ballasting operations must be suspended when entering a Ballast Tank. Ensure a
“Do not Operate” sign is posted on ballast / de-ballasting control board
- Smoking is prohibited, no smoking sign to be posted
- The Atmosphere in the space must be tested during Drager Tubes and Oxygen Meter prior to
entry
- Crew in the Enclosed Space must be equipped with a Personal Oxygen Meter(S), which must
be tested prior to entry
- Two SCBA’s and two EEBD’s must be on standby at the entrance to the enclosed space
- The Officer in charge should be monitor progress at all times

- A buddy system to be in placed; a person to be posted at the entrance


- Communication must be established and maintained between persons working in the space
and crew at the entrance

considered. In other words …"Is it still possible to execute the work or procedure with the controls
measures and procedures from stage 3 in place?"
