The project notification form is split into three separate parts to allow a clear structure for
information provided by the client, the fee proposal and then the appointment process if the fee is
accepted. Upon successful appointment, a request for information will then be issued for surveys,
drawings, designer details etc. If you have any questions whilst completing the form, please contact

CSS/PHS are pleased to be able to provide a fee CSS/PHS to detail service provision included

provision is accepted.

principal designer/CDM advisor on (project). By accepting the role of principal designer/CDM
advisor you confirm that you are competent to fulfill the role and have the necessary time and
resources to allocate to this project in line with the Construction (Design and Management)
Regulations 2015. By signing the client also acknowledges their responsibilities under these
regulations.
