Golden Memories is a full-service event planning company based in Lipa City.


Founded by Marife N. Patubo, who has over 2 years of experience in the industry.
We pride ourselves on our expertise, creativity, and attention to detail. We not only
organize extravagant event; we organize mesmerizing moments for you. Because
events may be forgotten but moments will last a lifetime.

Golden Memories, driven by passion and big ideas, provides customizes and
strategic event experiences where guest connect with our client’s brand and
products in ways that are personally relevant and memorable. Our company can
serve weddings, debuts, corporate events, gatherings and various other occasions
by providing dedicated service in the planning and management of their events
and additional support every way possible.

Our approach is mainly focused on the customer’s needs the quality of service.

Event Manger • She is the mastermind behind the entire operation.

promotion, overall co-ordination and profitability of
an event.

Assistant Event Manager • she/he is mainly responsible for assisting the event
manager in all aspects of event planning and

expended according to funders’ requirements.
• ensuring the formulation of the budgets according to
the directives and policies.

attendees and target attendees have all the
information they need to getting them there and
allowing them to have a great time.
Technical Committee • they are concerned with the preservation of sound
and audiovisual media and technically sustainable

before and during the event.

Facebook post announcing that your beloved daughter, Nicole M. Rivera, will celebrate
her success for passing Philippine Nurses Licensure Examination on the 28th of
February 2024. In line with this, we would like to send a proposal for the said event.

We, at Golden Memories, we plan a complete package that would suit to your need
and wants. We also offer other services such as event styling, photo and video
services, lights and sound systems, food carts and souvenirs through our suppliers.
Moreover, we would propose all the necessary decorations, venue, menu, program,
host and other materials to complete the celebration. This letter is not an official
contract agreement yet. All the terms and conditions of the proposed transaction would
be stated in the contract agreement, to be negotiated agreed, and executed by both
parties.

Event Concept and Description

EVENT THEME:

“Golden Compassion: Celebrating RN Success”


Motif: Black and Gold

A memorable evening celebration that honors the dedication, hard work, and
compassion of our registered nurse passer as she embarks on her journey in the
nursing profession. “Golden Compassion” symbolizes the combination of empathy,
dedication and excellence that defines the nursing profession. It represents the golden
standard of care provided by nurses and the compassion that guides her in their
practice.

The color black has the power to create sophisticated and elegant settings while
the color gold can instill value and success when incorporated into venue design. The
combination of the two colors can have a power effect on the overall ambience of the
venue. It is a warm and inviting color that can make the guests feel welcomed as well,
and does well in venue where the people often gather. Overall, a gold and black theme
for a board exam passer can symbolize their achievement, recognition and readiness
to excel in their chooses field.

To celebrate this memorable event, it will be held on February 28, 2024, 7:00
pm – 10:00 pm at La Vida Resort & Events Center located in front of Academia de
Lipa Purok 1 Barangay, Lipa City. Mostly, the celebration of this memorable event is
morning but for me as an event manager who wants to provide our clients with
unforgettable and personalized event experiences that exceed their expectations. The
event will be prepared in the evening. Since, our guest is not free in the morning the
client decides that the event will be start at 7:00 pm until 10:00 pm in the evening.

In essence, this concept aims to create a memorable evening event for our RN
passer, celebrating her achievements and also inspiring everyone as they transition
into her career as registered nurse. For the preparation of this event, here are the
design and layout for you to well-defined the concept:

• Entrance Area: Welcome signage or banner displaying the event name and
theme.
• Golden Spotlight Entrance: Welcome guests with a grand entrance featuring
golden spotlights or a black carpet or walkway leading into the venue.
• Décor and Atmosphere: The venue is adorned with golden accents and black
embellishments, evoking a sense of elegance and sophistication.
• Seating Arrangement: adorned with centerpieces featuring golden flowers and
black ribbons, symbolizing the blending of compassion and professionalism in
nursing. Round tables with chairs arranged throughout the venue for guests to
dine and enjoy the program.

• Stage Area: Welcome signage or banner displaying the event name and
theme. Podium for speakers to address the audience. Audiovisual equipment
for multimedia presentation and music.
• Gift and Merchandise Area: Designated space for displaying giveaway items
and merchandise related to the event.
• Cake Design: the cake features a tired design to symbolize the layers of
accomplishments and growth in the journey of becoming a registered nurse.
The color scheme predominantly includes gold and white, with accents of black
contrast.
• Recognition Wall or Display: Guests can browse the display and leave notes
of encouragement or well wishes for RN passer.
• Emergency Exits and Safety Measures: Clearly mark the emergency exits
and evacuation area. Ensure compliance with safety regulations and provide
first aid kits and personal site as needed.

CONTRACTS AND AGREEMENT

CLIENT AGREEMENT

THIS AGREEMENT is made this 10th day of January, 2024.

between Ms. Patrica M. Rivera and Golden Memories Events Services

with office at Carnelian St. Zone 3, Pangao Lipa City.

The Client wishes to be provided with the Services for the upcoming celebration for
her Unica Hija for passing the board exam as Registered Nurse happening on
February 28, 2024 by the Event Planner and the Event Planner agrees to provide the
Services to the Client on the terms and conditions of this Agreement.

1.1 Services
The Event Planner shall provide services in accordance with the terms and conditions
of this Agreement: Full coordination event planning on the upcoming celebration for
her Unica Hija for passing the board exam as Registered Nurse happening on
February 28, 2024 to the Client

VENUE
• Use of Air-conditioned La Vida Center
• Built-in Stage with Lifter, Basic Lights and Sound System
• Led Wall (basic)
• One (1) Guest Room
• One (1) dressing Room

1.2 Fees
As consideration for the provision of the Services by the Event Planner, the fees for
the provision of the Services is PHP 400,000.
The Client [shall/shall not] pay for the Event Planner’s out-of-pocket expenses
comprising [insert here, if agreed].
1.3 Payment
The Client agrees to pay the Fees to the Event Planner on the following dates:
PHP40,000 deposit after 30 days of contract signing. Another payment will be made
within 30-60 days prior to the first payment. The Event Planner shall invoice the Client
for the Services that it has provided to the Client [monthly/at the event/after the event].
The Client shall pay such invoices [upon receipt /within 30 days of receipt] from the
Event Planner.

1. For the above services, Client will pay Caterer an agreed upon price per head for
each meal.
2. Client may request additional services, with the knowledge that these services will
add to the total cost agreed upon by this contract, and must be agreed to in writing,on
a separate catering estimate.
3. The balance on the total cost for the Event is due upon arrival.

Any charges payable under this Agreement are exclusive of any applicable taxes and
such shall be payable by the Client to the Event Planner in addition to all othercharges
payable hereunder.
Food and SET-UP
The caterer agrees to provide the following services for the event
Menu:
• 4 COURSES (Pork, chicken, Fish, Pasta or Noodles oe Veggies)
• Dessert
• Rice
• Drinks
• Decorated Themed Backdrop
• Welcome signage board
• Themed table centerpieces for the guests table
• Elegant Design for the Venue
• Fiber buffet table set up
• Elegant chaffing dishes with food lamp
• Food labels for the buffet table
• Complete use of chinawares, silverwares, and glasswares
• Tiffany chairs and ribbon as per Motif
• Table setting for the cake and souvenir
1.4 Term and Termination:
This Agreement shall commence on the date first written above and shall continue
until the Services have been fully performed. This Agreement may be terminated by
either party upon [number] days' written notice to the other party in the event of a
material breach of this Agreement by the other party.
1.5 Insurance:
Contractor shall maintain general liability insurance and worker's compensation
insurance in amounts satisfactory to Client. Contractor shall provide Client with
certificates of insurance evidencing such coverage prior to the Event.
1.6 Indemnification:
Contractor agrees to indemnify and hold harmless Client from and against any and all
claims, losses, damages, liabilities, costs, and expenses (including reasonable
attorneys' fees) arising out of or in connection with the Services provided hereunder,
except to the extent caused by the sole negligence or willful misconduct of Client.

1.7 Force Majeure:
Neither party shall be liable for any delay or failure to perform its obligations hereunder
due to causes beyond its reasonable control, such as acts of God, war, terrorism,
strikes, or natural disasters.

1.8 Warranty
The Event Planner represents and warrants that it will perform the Services with
reasonable skill and care.
1.9 Limitation of Liability
Subject to the Client’s obligation to pay the Fees to the Event Planner, either party’s
liability arising directly out of its obligations under this Agreement and every applicable
part of it shall be limited in aggregate to the Fees. The Event Planner assumes no
liability due to the quality of items or services purchased for the Client.
2.0 Term and Termination
This Agreement shall be effective on the date hereof and shall continue until the date
of the event unless terminated sooner. If the Client terminates this agreement for
anyreason more than 30 days before the scheduled event, the Client will forfeit the
deposit paid to the Event Planner and the Client shall reimburse the Event Planner for
all outstanding out of-pocket expenses. If the Client terminates this agreement for any
reason within 30 days of the scheduled event, the full fee is payable to the Event
Planner and the Client shall reimburse the Event Planner for all outstanding out-of-
pocket expenses.
2.1 Relationship of the Parties
The Parties acknowledge and agree that the Services performed by the Event Planner,
its employees, sub-contractors, or agents shall be as an independent contractor and
that nothing in this Agreement shall be deemed to constitute a partnership, joint
venture, or otherwise between the parties.
2.2 Confidentiality
Neither Party will disclose any information of the other which comes into its possession
under or in relation to this Agreement and which is of a confidential nature.
2.3 Miscellaneous
The failure of either party to enforce its rights under this Agreement at any time for any
period shall not be construed as a waiver of such rights. If any part, term or provision
of this Agreement is held to be illegal or unenforceable neither the validity nor
enforceability of the remainder of this Agreement shall beaffected.
This Agreement constitutes the entire understanding between the Parties relating to
the event and supersedes all prior representations, negotiations or understandings
with respect to the event.

Neither Party shall be liable for failure to perform any obligation under this Agreement
if the failure is caused by any circumstances beyond its reasonable control, including
but not limited to acts of god, war, or industrial dispute.

This Agreement shall be governed by the laws of the jurisdiction in which the Client is
located.

The balance (25%) of 25,000 will be paid in two parts. The sum of 6,250 will be paid
at the end of the event and the balance of 18, 750 upon delivery of video/photos.
Terms and Conditions

1. Picxellence is the exclusive Videographer and/or Photographer retained by the


client to cover outlined event. Any conflicts with other photographic and/or video
coverage contracts and any notifications necessary to avoid such conflicts are the sole
responsibility of the client.

Client assumes all responsibilities for obtaining any necessary permission, clearance
permits, etc., which may be required for Picxellence to photograph/video each event,
public or private. Client assumes all responsibility for obtaining and retaining
permission for access to any requested camera positions and is solely responsible for
the quality of the final production resulting from the use of, or inability to use, such
camera positions. The client warrants that he/she has the legal rights to anything
Picxellence will videotape, including photos, musical recordings, videotapes, or any
other materials delivered to Picxellence for inclusion in the client’s videotape or DVD.
Picxellence reserves the right to use videos and photos for display and or promotional
purposes. All video masters, raw footage and photographs remain the exclusive

Artistic Style. On its own behalf and on behalf of the subject: Client acknowledges
that he/she is familiar with the Company’s portfolio and is requesting services with
knowledge of the company’s style and that the Company’s work is constantly
evolving; that the Company's services are of a unique and artistic nature; that the

photos and or video may be different from photos and or video done by the
company in the past; and that in creating the photos or videos we shall use our
own creative artistic judgment to create images consistent with our personal
judgment to create images and videos consistent with our vision of the event,
which may be different from the clients and or the subject’s vision of the event.
Accordingly, client acknowledges that the photos or videos shall not be subject to
rejection on the basis of taste or esthetic criteria.

1. The client reserves the right to terminate the contract for any reason up to 30
days after the date of downpayment. Should the client terminate the contract,
all money paid minus the retainer which is 25% of total cost will be
refunded.
2. If Pixcellence fails to comply with the terms of this agreement due to any event
or at beyond the control of Golden Memories Events s, including but not
limited to illness, accident, labor disputes, acts of God and other catastrophes,
or uncontrollable and unforeseeable equipment failure Picxellence liability is
limited only to a refund of all monies received.
3. Picxellence produces DVDs using the most compatible media for set top
players (DVD-R). Picxellence is not responsible for incompatibility with the
DVDs created and the client’s player.

HOST AGREEMENT
This Agreement is made and entered into as of February 07, 2024 by and between Mr.
Dexter Vinagrera, “

WITNESSETH
WHEREAS, Host desires to host the La Vida Resort Event and Center on February
28, 2024, and WHEREAS, Event desires to have Host the Event.
NOW, THEREFORE, in consideration of the foregoing premises and the mutual
covenants contained herein, the parties agree as follows:

1. Host agrees to
• Obtain all necessary permits and licenses for the Event.
• Comply with all applicable laws and regulations.
• Maintain the venue in a clean and safe condition.
• Provide adequate security for the Event.

• Be responsible for any damage to the venue caused by the Event.


2. Event Responsibilities

2.1 Event agrees to:


• Provide all necessary marketing and promotional materials for the Event.
• Secure all necessary vendors and suppliers for the Event.
• Manage all ticketing and registration for the Event.
• Provide insurance for the Event
• Be responsible for any damage to the venue caused by Event attendees.
3. Payment

3.1 Host will be [compensated/not compensated] for hosting the Event. [If

4. Cancellation

4.1 Either party may cancel this Agreement upon 10 days' written notice to the other

to the subject matter hereof and supersedes all prior or contemporaneous

PASTRIES AND GRAZING TABLE AGREEMENT

This Agreement is made and entered into as of February 13, 2024, by and between
Golden Memories Events Services, with a principal place of business at Lodlod, Lipa
City and. Sweet N’ Courner
WHEREAS, Client desires to engage Provider to provide pastries and grazing table
and desires to provide such services on the terms and conditions set forth herein.
NOW, THEREFORE, in consideration of the mutual covenants contained herein, the
parties agree as follows:
1. Services Provider shall provide and assemble a grazing table for the Client's event
on February 28, 2024, from 7 pm to 10 pm. The grazing table will include the following:
• Brownies, Chocolate Dip, Cupcakes, Pretzels, Bread etc.
2. Fees and Payment Client shall pay Provider a total fee of 10,000 for the Services
of the Grazing table and Php 5,000 for the pastries. The fee shall be payable as
follows:
• February 24, 2024
3. Cancellations and Refunds
(a) Client may cancel this Agreement without penalty up to 20 days before the event
date. In the event of such cancellation, the Provider will refund all payments made by
the Client.
(b) If the Client cancels 14 days before the event date, the Provider will retain 50% of
the total fee and refund the remaining 50%.
(c) If Provider cancels due to unforeseen circumstances, such as inclement weather,
natural disasters, or supplier shortages.
Provider will refund all payments made by Client.
(d) In the event of a partial cancellation of the Services by the Client, the fee will be
adjusted accordingly.
4. Delivery and Setup
Provider shall deliver and set up the grazing table at the event location at 7 pm. The
client shall provide access to the event location and any necessary assistance with
setup.
5. Clean-up and Removal
(a) If included in the Agreement, Provider shall remove all rental items and dispose of
any unused food items after the event.
(b) If the Client is responsible for clean-up, the grazing table area must be left clean
and free of debris.
6. Liability and Insurance

Provider shall maintain liability insurance covering its operations and the Services
provided under this Agreement. The client shall be responsible for obtaining any
necessary event insurance.

7. Force Majeure
Neither party shall be liable for any delay or failure to perform its obligations under this
Agreement due to causes beyond its reasonable control, including acts of God, war,
terrorism, strikes, floods, fires, or other natural disasters. 8. Entire Agreement This
Agreement constitutes the entire agreement between the parties concerning the
subject matter hereof and supersedes all prior or contemporaneous communications,
representations, or agreements, whether oral or written
