    On September 13, 2021, a Tata Steel employee injured his left ankle while getting down from a dumper truck near a scrap dumping area. He was inspecting the dumper truck after his shift when he lost his balance and fell, injuring his ankle. He was taken to the on-site medical center and then to a hospital for treatment. An investigation found that the flooring at the incident site was unsafe. As a result, inspections will now be conducted only at designated areas and employees are being reminded to be attentive when entering or exiting vehicles.

    Injury : Swelling on left ankle.
    What happened:
    On 13th September 2021, the injured person (IP) was in ‘C’ shift duty. He went to inspect the Euclid Dumper
    No - 137 which was parked near LD#3 scrap dumping area. After inspection, while he was getting down
    from the dumper, he became unbalanced resulting in an injury to his left ankle. IP was immediately taken to
    the East Plant First Aid Centre and therefrom referred to TMH for further treatment.

    Preliminary Findings:
    1. IP was following the three-point contacts rule while getting down from the vehicle.
    2. Floor condition at the incident site was found unsafe.
    3. Illumination at the incident site was found adequate.

    Immediate actions taken:


    1. Incident was communicated to all concerned employees.
    2. Incident was logged in ENSAFE (PIR - IN00124126) system for detailed investigation.

    Recommendations:
    1. Inspection of the vehicle should be conducted at the designated place of the customer site.
    2. All employees should be attentive while getting up/down from the vehicle.
