    Nkulu Mutombo Gloire suffered an abrasion to his right knee after slipping and falling while moving a water bowser trailer with a coworker. The incident occurred on August 31, 2023 at 08:30 at the Smelter Main Warehouse. An investigation found that Gloire was wearing proper PPE but the ground was wet and slippery. As an immediate action, Gloire was taken for medical treatment and an investigation was conducted. The preliminary cause was determined to be a slip and fall due to slippery ground conditions.

    At approximately 08h30 M. Nkulu Gloire and his co-worker was tasked to
    move a water bowser trailer and to hook the trailer to a vehicle. Whilst
    Description of incident:
    (What/How) performing this task he slipped and fell which caused an abrasions to his
    right knee.

    Findings During Investigation:
     Wet and slippery ground conditions were found at the incident
    area.

     The injured was taken to the clinic for medical treatment.
     An in loco investigation was conducted immediately.
    Immediate Action taken  Incident was discussed with crew and communicated to all relevant personal.
     Responsible persons were notified.

     Poor Judgement – a vehicle should be used to move trailers etc.
