Terms and Conditions

I / We are aware that:


1. I agree to inform the service in writing immediately of any changes to the above information
2 Full fees are payable once the child's allowable absence is consumed
3. Two weeks fee payment in advance is held at all times
4. Four weeks written notice must be given if you wish to change a child's days of attendance or remove them
from the service
5. I agree to pay the enrolment fee of $150 prior to my child starting, and am aware that the enrolment fee is
non-refundable
6. The service has a right to restrict the method of payment to direct debit only
7. Any account that is declined will be charged an additional fee of $15 per week. If an account is declined for
two consecutive payments, your position will be reviewed and possibly cancelled
8. I agree to keep my fees paid up to date and understand that my child's position at the service will be in
jeopardy if my fees are not kept up to date
9. I understand that all booked days are paid for even when my child is absent due to sickness, holidays, public
holidays or any other kind absence
10. I understand it is my responsibility to ensure I understand my childcare subsidy entitlements and to keep
MyGov up to date with any changes. Full fees may result if any CCS entitlements are cancelled or not
retrieved by our service
11. I understand if I don't pay my child's fees, the service has the right to call upon a debt collector to obtain
fees. With this, in accordance with the Privacy Act 1988, I authorise the service to give my information as
may be required in response to the defaults of my payments. I also understand that I shall be liable to pay
for collection costs and late payment fees that will be added to my outstanding balance if do not pay within
the specified time
12, I am aware that if my child has not been collected by closing time and I am unable to be contacted, those
persons nominated will be called to collect my child
13. I agree to pay a late fee atter closing time. In the event that a child is left at the service for over an hour
after closing and service staff have been unable to contact anyone to collect the child, Educators or the
Nominated supervisor may be required to contact the Police while awaiting your arrival. In this instance,
the Service is also obligated to notify relevant Child Protection agencies and or the Regulatory Authority
14. I understand that in accordance with our conditions of funding with the Department Of Family and
Community Services, we are required to abide with a priority of access order as listed:
a. First Priority: A Child at risk of serious abuse & neglect
b. Second Priority: A Child of a single parent who satisfies, or of parents who both satisfy
the work/training/study test under section 14 of the Family Assistance Act
c. Third Priority: Any other child
d. Please be advised that in certain Circumstances we may need to adjust attendance pattern of your Child
to meet our funding requirements with commonwealth priority access.
15. Educators have a legal responsibility as mandatory reports to report any possible type of abuse and
neglect. Educators have duty of care to protect children from all forms of risk and harm
16. I agree to follow, support and abide by the service policies, procedures and practices. I am aware the
service is available to discuss any further policies should I need further clarification

Privacy Act 1988
• The family warrants that no bona fide dispute exists in respect of the debt & the family will indemnify the
service against any claims arising from any action taken.
• The service warrants that all information provided to collector regarding a debt has been collected In
accordance with the principles contained in the privacy act 1988 and that the debtor has authorised the
use or disclosure of that information in any way deemed necessary by the service for the purpose of
collecting the debt from the debtor.
• The family authorises the collector to the extent permitted by law to collect, retain and use any information about
the family, for the purpose of assessing the credit worthiness of the family.
• The family authorises the collector to disclose any information obtained to any person for the purposes
set out in the above point.

Default & Consequences of Default


• If the family defaults in payments when due. the family shall indemnify the service from and against all
legal costs and disbursements for collection or attempted collection amount calculated on a solicitor and
own client basis.
• If for any reason fees are not paid within given notice the service reserves the right to cancel & replace
days of care.

If the family does not comply with the service’s terms of payment the service reserves the right to cancel &
replace days of care.

Privacy Disclaimer
We acknowledge and respect the privacy of its clients. The enrolment information that is collected assists us to
meet our legislative obligations and provide the best level of education and care for your child. By completing this
form, you have consented to this information being collected. The information will be used by educators/staff
members and relevant government authorities. You have the right to access and alter personal information
concerning yourself or your child in accordance with the Privacy Act 1988 and our Privacy and Confidentiality
Policy.

I acknowledge that the parent handbook will be provided to me upon enrolment and I will follow all details
outlined within this handbook.
