SINUMPAANG SALAYSAY
Akong si IMELDA C. YANGA, may sapat na gulang, Pilipino at naninirahan sa

Gatbuca, Calumpit, Bulacan, matapos makapanumpa ng naaayon sa batas ay malaya at kusang
loob na dito ay nagpapahayag:
1. – Na ako ang asawa ni EMMANUEL A. YANGA na namatay noong Hunyo 18, 2010
sa Gatbuca, Calumpit, Bulacan;
2. – Na dahil sa kanyang pagyao, ako ay nagkagastos ng halagang P19,000.00 para sa

bayad sa funeral service ng Candido Memorial Homes at halagang P6,700.00 para naman sa mga
gamit at pagkain sa aming ginawang lamay o burol para sa kanya, ang kabuoang halagang
nagastos ay halagang 25,700.00;
3. – Na aking hinihiling sa SSS na mabigyan ng burial benefits o anumang benepisyo

ayon sa umiiral na batas ang pagyao ni EMMANUEL A. YANGA;
4. – Na isinagawa ko ang salaysay na ito upang patotohanan ang lahat na dito nasaad.
SA KATUNAYAN NG LAHAT, ako ay lumagda sa ibaba nito ngayong ika-1 ng Hulyo,

2010 dito sa Malolos, Bulacan.

ayon sa umiiral na batas ang pagyao ni ZHERRLE D. FRAC.

We, EDUARDO DE JESUS and BERNAR D. FAJARDO, both of legal

ages, Filipinos and residents of Brgy. Poblacion, Calumpit, Bulacan, under
oath, hereby depose and say:
1. We personally know the late DIOGENES MACAM MAKAPUGAY,

JR. who died on April 21, 1995 at San Fernando, Pampanga;
2. That said deceased was buried in their house in Poblacion,

Calumpit, Bulacan for about four (4) days;
3. That we know these facts because we were there and present during
the burial mass in Calumpit Church, Poblacion, Calumpit, Bulacan;
4. That we will also present at the time of the interment of the

deceased in Calumpit Catholic Cemetery on April 25, 1995;
5. That we know such facts for the reason that we are their close
friends and barriomates:
6. That, we are not in anyway related with each other even by affinity
or consanguinity; and
7. That by these presents, we hereby certify for the truthfulness and

veracity of the above mentioned facts.

I, SONIA MARXYZ C. PADILLA-VARGAS, of legal age, married,

Filipino and with residence and postal address at Gatbuca, Calumpit,
Bulacan, after having been duly sworn to in accordance with law hereby
voluntarily depose and say:
1. That, I am one of the four (4) children of SONIA CRUZ PADILLA, who is
presently in the United States of America (USA);
2. That SONIA CRUZ PADILA is the lawful owner of a Cemetery Lot

located at Pen-Nel Memorial Gardens and Services, Inc., Calumpit,
Bulacan identified as Sec. 7 Blk. 2 Lot 4-a;
3. That I am also the niece of the late CRESENCIA C. CRUZ who died on
November 22, 2018 and due for interment on November 25, 2018 at
Pen-Nel Memorial Gardens and Services, Inc., Calumpit, Bulacan;
4. That my mother SONIA CRUZ PADILLA consented to the burial of

CRESENCIA C. CRUZ in the cemetery lot owned by my mother SONIA
CRUZ PADILLA, as per attached email and letter-authority;
5. That I am executing this affidavit to attest to the veracity of the facts

contained herein and for whatever legal intents and purposes it may
serve.
