That sometime on Nov. 15, 2002 my friend Jose requested me to accompany him to San
Fernando City, Pampanga;

That on our way to our destination or while within the vicinity of United Pulp and Paper
Co. (UPPC), Brgy. Palimbang, Calumpit, Bulacan a male person later on identified as Edilberto
Dizon was accidentally hit by the vehicle being driven my Jose;

 That in connection with the incident, I earlier gave my statement before the Investigator
of Criminal Investigation Service at Malolos City. A copy of which is hereto attached and made
