    We, ESTRELLA V. PULGA and LOLITA V. BOLUS, both f legal age,


    married, Filipinos, and with residence & postal address of Blk 113 Lot 4, Sta.
    Lucia, Dasmarinas City, Cavite, and Blk-113 Lot 1, Sta. Lucia, Dasmarinas City,
    Cavite, after having been duly sworn to in accordance with law, hereby depose
    and say:

    1. That we are the family friend of ANACLETO JABIEN TUAZON, who


    was born on November 10, 1944, in Jiabong, Samar (western
    Samar);

    2. That he is son late spouses SEVIRINO TUAZON and FRUCTOSA


    JABIEN, of and resident of Jiabong, Western Samar;

    3. That we are executing this affidavit for legal purposes of proving


    that the reason for the delay in registering her Birth Certificate
    causing the late released of signing the document.

    IN WITNESS WHEREOF, we have hereunto set our hands this 29th of May
