Service Support Animal Addendum REV 2
As a reasonable accommodation under applicable state and federal law, Owner/Agent has granted the above named resident(s)
request for a revocable license to permit the retention of a service/support animal. Resident understands that there is no additional
fee or deposit required, but agrees to the following:
1. Subject to compliance with the terms of this Revocable License, the following service/support animal has been
approved to reside in the unit:
2. Service/support animals must be properly licensed with required shots and vaccinations.
3. For transportation on and off the property, all service, support and companion animals must be carried or walked on a
short hand-held leash. No leash may exceed six feet in length and no animal may be leashed to any stationary object on
the common areas. Leaving a service/support animal unattended on deck or patio so as to disturb other residents is
strictly prohibited.
4. No service/support animal shall be left unattended in the unit for more than 12 hours.
5. Service/support animal owners are responsible for the immediate removal and proper disposal of kitty litter, other
comparable litter and any other animal waste on all areas of the property. Such waste and litter will be appropriately
and tightly sealed in a plastic bag and disposed of in the dumpster. Under no circumstances shall litter be disposed of in
the toilets or down other drains.
6. Service/support animals must not be left unattended outside the unit
7. Service/support animals must only be fed in their apartment.
8. Resident must keep its apartment clean at all times – including, but not limited to, free from fleas and odors.
9. Service/support animal owners are responsible for any property damage, injury or disturbances that their animal may
cause or inflict. Resident must have an up to date insurance certificate on file at the management office.
10. Commercial breeding of animals on the property is strictly prohibited. No animal shall be kept or maintained for
commercial purposes.
11. No service/support animal owner shall inflict or cause cruelty in connection with any animal
12. Upon vacating the apartment, Management shall do an inspection of the apartment and any damage (including, but not
limited to, carpet, padding and other flooring) attributed to the service/support animal shall be charged to the resident
accordingly.
13. Resident shall insure that service/support animal does not disturb other residents on the property, Management
employees, or its agents at any time. As compliance with this requirement is the essential underlying basis for
reasonably accommodating Resident's request for a service/support animal, breach or violation of this requirement may
result in revocation of the this license and may also form the basis for termination of the underlying lease if any
service/support animal breaches or violations are not timely remedied or cured.
14. Except for the service/support animal described above, Resident shall not keep any other pet or offspring of this
service/support animal on the property.
15. In case of an emergency (i.e. if a service/support/companion animal becomes vicious or displays symptoms of severe
illness; or if the service/support/companion animal owner is taken to the hospital or becomes deceased and has failed to
provide for the care of said animal or the designee fails to care for the service/support/companion animal), then
Management reserves the right to contact the proper authorities to have said animal removed without prior notice to
Resident. In such situations, Management will not be liable for any steps taken by it or by the proper
authorities. Resident shall be responsible for any fines, fees or related expenses as a result of the service/support
animal’s removal.
16. The Resident is responsible for reasonable charges as a result of the service/support animal including but not limited to
attorney’s fees, court fees, or other related expenses which may be incurred to correct any service/support animal
violation. If at any time there are charges incurred as a result of the animal, and payment of said charges are not paid
by the Resident, Management will deliver a ten (10) day notice of past due charges. If at the end of ten (10) days, the
past due charges are not paid, Management reserves all rights and remedies available to it by law and may also revoke
this service and support animal license.
Failure to comply with any of the terms of this Agreement (which may be amended from time to time) shall constitute a default
and will result in license revocation and/or lease termination at Management’s discretion.
I have read, understand and agree to the above Service/ Support Agreement Addendum to the Lease Agreement in its entirety. I
also understand that failure to adhere to the terms of this Addendum to Lease and the Guidelines in their entirety will result in
license revocation and/or the termination of my tenancy.
