Pet Boarding and Daycare Contract
This pet boarding and daycare contract ( shall be referred to as “Contract” hereinafter) is
entered into by and between the Casa Del Misfits Pet Boarding and Daycare (“Company”
hereinafter).
1. Information of the Pet

2: Services and Service Fee

The service to be provided under this contract is essentially an accommodation service for
the Pet. Service fees are to be paid upon pick up of the pet. The Owner is obliged to deliver
all food, medicine and any additional special supplies that the Pet will require during the
time of boarding, to the Company during the delivery of the Pet.
3: Risks, Emergency Services and Veterinarian

The Owner understands that there are risks associated with an off leash, cage free
socialization environment for the Pet. The Owner agrees to such risks and accepts these
risks. Owner understands that while socialization and play in monitored to prevent injury, it
is still possible that during the course of normal socialization and situations may arise that
require urgent medical intervention.
Regardless of whether it is in the natural flow or due to socialization, if it is determined that
the Pet needs urgent medical attention, the Company will contact directly with the
veterinarian specified by the owner below.

The Owner agrees, declares and undertakes to pay all transportation and medical
intervention costs to the Company upon first request in cases where urgent medical
intervention is required.
If the above mentioned veterinarian cannot be reached, the Company may also take the Pet
to a veterinarian of its choice, with verbal consent of the Owner. In this case, all costs are
also paid by the Owner. Records and transcripts of all interventions made are presented to
the Owner and a copy is provided upon request.
4: Term of the Agreement

The Agreement shall become effective as of the date of its execution and shall continue in
effect until such a time that the Owner or Company part ways.
The Pet can be picked up by someone authorized by the Owner. This person must present
signed authorization or verbal communication from Owner with proper Identification.
Otherwise, the Pet will not be handed over to a third party.
5: Mandatory Documents to Submit

In order to prevent contagious diseases, it is obligatory to submit the Pet’s medical history
and vaccination record of the Pet to the facility. All record must be veterinary approved.

6: Advertising
The Owner knows and accepts that the Company may record videos and take pictures of
the Pet and may use these records as a marketing asset on marketing materials, website,
and/or social media channels of the Company. The Owner gives permission to use any video
or photographs taken of the Pet in any future marketing efforts whether online or in print.
7: The Owner acknowledges that the Owner has hereby been notified by the Company that
if the Pet is not retrieved by the Owner within ten (10) days of the Pet’s scheduled Pick-up
date, then the company may deem the Pet as “abandoned” as the regulation of the
applicable laws. The Company may then relinquish custody of the Pet to the nearest animal
shelter in the area or take such other action as permitted by applicable laws. Despite any
such abandonment, the Owner will remain liable to the Company for all unpaid services and
care charges.
8: Miscellaneous Provisions
The Agreement shall be governed by and construed in accordance with the laws of the State
of Ohio. In the event of any dispute relating to the conclusion, validity, the implementation
or the interpretation of the Contract, the courts of Cuyahoga County will have sole and
exclusive jurisdiction.
This Contract contains the entire agreement between the parties. All terms and conditions
of this Contract shall be binding on the heirs, estates, assigns and successors of the parties
The Company reserves the right to turn away any animals at check-in or also contact the
Owner to pick up the Pet if it is determined that the Pet is a risk factor for other animals in
the facility.
