MRI RT Tibia without Contrast
CLINICAL INDICATION: 7 months pain TECHNIQUE: Multiplanar multisequence MRI of the [right] proximal tibia was performed without IV contrast according to standard departmental protocol. COMPARISON: No previous MR examination is available for comparison. No plain radiograph was available for comparison. FINDINGS: A large, eccentric, partly defined, multiloculated, mildly expansile, intramedullary lesion is identified in the right proximal tibia, at metadiaphyseal junction. Lesion is appearing iso intense on T1 with internal areas of hyperintensity, excessively hyperintense on T2. Post contrast enhancement cannot be commented upon due to study limitations. No perilesional soft tissue component or frank cortical breach is seen. Lesion is also not reaching the articular surface. Lesion is showing multiple internal air fluid levels. At the point of maximum dimension, this lesion is measuring 55 x 34 x 29 mm in CC, TS and AP dimensions. Rest of the visualized muscles and bones return to normal signal. Neurovascular bundle is intact. Intact articulation of right knee joint. Rest of visualized bones return normal signals. IMPRESSION: Overall appearances are favoring aneurysmal bone cyst. Another less likely differential is telangiectatic Osteosarcoma. Correlation with plain radiography and contrast enhanced MR imaging is recommended.
