more above the floor level, measured from the finished floor level, is considered work
at height. When the floor is a platform, such distance shall be counted from the
platform, provided it has adequate handrails or other means of protection so that the
worker is not at risk of falling off.
16. List the safety equipment that must be worn by workers assigned to work at heights ?
Personnel must wear during all activities within the work area, the complete safety
equipment required by regulations; helmet with chinstrap, safety glasses, cotton
clothing, shoes, earplugs, leather and/or dielectric gloves and during work at heights,
wear a safety belt or harness with a life cable (nylon or polyester 13mm, 13mm), safety
goggles.During work at heights, wear a safety belt or harness with life line (nylon or
polyester 13mm, ½" ø), reference Annex 4. The selection of the safety system for
working at heights must be made by means of a risk analysis to be carried out by the
work group. Those responsible for supervision during the execution of the work and
each user of the safety harness must check that it is in good condition just before
putting it on, and must also wear the corresponding lifeline, making sure that the
anchorage point selected for anchoring is above the level of his or her head.
Delimit restricted access areas due to the presence of hazards, excavation work, work on electrical
registers, cutting and welding work (hot work), work with chemical hazards, etc.
18. In which activities would you place red safety barriers with white lettering?
It is used to delimit an area where entry is restricted to all personnel (prohibitive sign, stop,
firefighting equipment and systems), except when specific permission has been granted by the
supervisor or middle manager in charge of the area.
19. What type and color of safety barrier would you place to delimit the work in a radiographic
inspection and at what distance would it be placed?
The tape with yellow safety color and magenta contrast is used to warn of the presence of ionizing
radiation. When radiographic inspections are performed, this tape should be placed covering a
minimum radius of 15 m with respect to the radiation source.
20. Would you start a job that requires the placement of safety barriers and this has not been
installed, explain?
No, because it is required to comply with the pertinent safety measures for the execution of the
work.
21. Mention the types of safety barriers that are used in this work center and give an example of
each one.
Safety barriers: Temporary fences or removable railings, used to delimit restricted access areas
due to the presence of risks.
Tape-type safety barrier: Plastic device in the form of a rolled-up tape used to delimit risk areas.
Trestle type safety barrier: Metal, wood or plastic device, resistant to impact.
Cone type safety barrier: Device made of PVC used to delimit vehicular traffic.
Perimeter fence type security barrier: Prefabricated device based on posts and barbed wire, with
access gate, tapes and restrictive signs to delimit excavation areas in the right-of-way.
26. Mention the basic safety equipment for cutting and welding:
Welding masks or goggles with welder's shade, face protection, hoods (nuns), fume respirator,
leather gloves with mittens, leather apron, coat, canvas gaiters, sleeves, special masks with 10 to

protection with shading 4 to 8 and, in special cases, a face shield with dark glass.
27. What is the % of explosivity that you should mark before igniting any ignition source?
When performing the explosivity test, the result should be negative (o%) before igniting any
source of ignition, in equipment or machines.
28. What safety measures should you consider when performing cutting and welding work?
All technical and manual personnel involved in the cutting and welding activity must comply with
Safety, Health and Environmental Protection regulations before, during and after their work.
Before starting the cutting and welding, it must be physically verified that the conditions
requested in the work permit, the checklist, the double checklist and the AST are met at the site
where the activity is to be performed.
When cutting and welding, operators must wear leather gloves with mittens, leather coat
hangers, leather jackets, canvas gaiters and special shaded face shields from 10 to 14for
protection . Helpers should be provided with gloves and eye protection with shading from 4 to 8.
Operators and assistants involved in the activity must be physically and mentally fit to perform
the work.
Before starting cutting and welding work, fire protection must be in place. This fire protection
must remain in place until the cutting and welding work is completed.
When process lines are cut or welded, they must be purged, insulated with blind joints, blocked,
vaporized, inerted and depressurized according to the procedure "Opening and closing of flanges
and connections in lines and equipment".
Cylinders must be placed at a minimum distance of 6 meters from the work site in a vertical
manner; they must also be secured to prevent them from falling or being knocked.
Perform explosivity test with negative result (o%) before igniting any source of ignition, in
equipment or machines.
Communicate to all personnel in the area involved about the execution of the cutting and welding
work.
Locate and have chemical powder or CO₂ extinguishers, previously reviewed and tested by safety
personnel, near the workplace.
All welding machines that use electrical power must be grounded to earth before starting
operation.
The work cables should not wrap around or over the welding generator during work, as this may
cause unfavorable effects on the magnetic fields of the machine and instability of the welding arc.
All cables or hoses of machines and equipment for cutting and/or welding work must be in perfect
condition for use, without splices or joints.
All gas, oxygen, acetylene or argon cylinders must be transported in diablo or devil's
wheelbarrows with rubber tires (not metallic), secured with chains to prevent them from falling,
with a cap installed and with the risk classification diamond labeled or printed.
The operating personnel must deliver the area where the cutting and/or welding activity is to be
performed clean and clear.
No worker should look at the electric arc without the correct eye protection. Welders should
invariably use screens, fixed or portable, when welding with electric arc.
If sampling is required for special reasons, cutting and/or welding activities should be momentarily
suspended during sampling. To restart cutting and/or welding work, the negative explosive
conditions (o%) must be met again.
Drums should not be used as tables for cutting and/or welding work.
Temporarily interrupt welding work when there is danger of contact with water (excessive
perspiration, abundant water or rain).
At the end of the cutting and welding work, the maintenance personnel must hand over the clean
and clear area to the operating personnel.

equipment?
This is the form that must be filled out and processed in order to authorize the performance of
potentially hazardous work.
35. Who are the parties involved in filling out a work permit?
It is for a maximum of 12 continuous hours and may be revalidated for two consecutive 12-hour
periods. In the event that work is suspended or interrupted, it may be revalidated as long as such
interruption or suspension does not exceed 24 hours, otherwise, a new permit must be generated.
38. Who must generate the work permit when the activity is to be performed by a contractor?
ORIGINAL: It is for the Executor of the Work, who must place it in a visible place in the place where
the work is to be carried out and during the time it is being executed (in a transparent plastic bag,
so that it does not deteriorate).
Upon completion of the work, the original of the Work Permit must be exchanged with copy No. 1,
so that the Operational Manager keeps the original and the Work Performer keeps the copy No. 1,
so that the Operational Manager keeps the original and the Work Performer keeps the copy No. 1.
1.
COPY NO. 1: It is for the Operational Manager, who shall place it in a visible place in the place
where the operating personnel in charge of the installation or equipment works, and shall inform
in writing to the Central Control Room Operator, for the knowledge of all the personnel in the area
until the work is completed.

By suffocation: This method is intended to eliminate oxygen. For this purpose, blankets are
used, earth is thrown in, etc. The earth should be applied by throwing it all at once on the
base of the flames; when there is little, it is preferable to pile it up so that it can be thrown
by shovelfuls. Special foams used for hydrocarbon fires also act in this way.
By dispersion or isolation of the fuel: This method prevents the spread of fire by placing
barriers so that the fire does not reach more combustible materials. Firebreaks, or cutting
vegetation before the fire reaches a forest fire, are the most commonly used.
By inhibition of the chain reaction: With this method, an attempt is made to cut off the
chain reaction, for which chemical substances are used. Powder chemical and halon
extinguishers operate by this method.
