the south side of UST-3, the west side of UST-6, the north side of UST-7, and the east side of UST-8
(Figure 3, Table 3, Appendix A) suggesting that these tanks may have leaked.
• None of the analytical results from soil samples collected from below product lines contained
concentrations of TPH-GRO or TPH-DRO above UST Action Levels (10 mg/kg) (Figure 3, Table 3,
Appendix A).
• No volatile organic compounds (VOCs), semivolatile organic compounds (SVOCs), extractable
petroleum hydrocarbons (EPH), or volatile petroleum hydrocarbons (VPH) hydrocarbon fractions
exceeded Soil-to-Groundwater (GW) Maximum Soil Contaminant Concentrations (MSCCs) in the soil
samples collected at 10-foot intervals during the installation of the “source area” monitoring well
MW-2 (Figure 2, Table 4, Appendix A).
• Apparent groundwater flow direction is to the southwest (Figure 4). However, only three monitoring
wells were used to measure potentiometric surface elevations, which can be locally variable.
Therefore, the estimated groundwater flow direction is not necessarily representative of the entire
site. Based upon surface topography and features, groundwater had been expected to flow toward
the southeast.
• No constituents were detected above the North Carolina Ground Water Quality Standards (NC GWQS)
in groundwater samples collected from monitoring wells MW-2 and MW-3. Four VOCs were detected
above NC GWQS in monitoring well MW-1, including 1,2-dichloroethane, 1,2-dichloropropane,
benzene, and diisopropyl ether (Figure 2, Table 5, Appendix A). These constituents are not
commonly associated with USTs and may have originated from the site’s septic tank or the recessed
work areas in the garage, which are located upgradient of monitoring well MW-1.
Based on the constituent concentrations detected in soil and groundwater, a receptor survey will be
conducted by Altamont and submitted under a separate cover. The results of the receptor survey and the
site groundwater and soil analytical data will be used by DENR to determine the risk level of the site and
whether additional site activities will be required by DENR in the future.

push machine, or using a decontaminated stainless steel hand auger. Disposable nitrile gloves were also
used during sampling activities and were changed in between sample locations to prevent sample cross-
contamination.
Soil samples were collected in accordance with the Guidelines for Site Checks, Tank Closure, and Initial
Response and Abatement for UST Releases (DENR 2008b). Soil samples were collected from borings
located around the perimeter of a single UST or around the perimeter of a set of closely spaced USTs. These
samples were collected within three feet of the USTs at the depth of the tank bottom, or no deeper than two
feet into native soil below the depth of the tank bottom. Soil samples from locations near USTs were
collected from seven to ten feet below ground surface (ft-bgs), depending on the depth of the tank. Soil
samples are designated with an “SB-X” nomenclature, with the “X” designating the boring number. One soil
sample was collected for every ten feet of UST, on each side of a UST. However, soil samples could not be
collected from the south side of UST-7, the east side of UST-3, or the west sides of UST-1 and UST-2, due to
the proximity of the site building, the two-bay garage, and a sewer line, respectively.
Subsurface soil samples were also collected adjacent to and beneath identified product lines. Product line
samples were collected at a depth of 2-3 ft-bgs and are designated with a “SB-PL-X” nomenclature, with the
“X” designating the boring number.
In addition, one surface soil sample was collected at each of the fill ports identified at the subject property to
evaluate the potential of past overfills. These samples were designated with a “SB-FP-X” nomenclature, with
the “X” designating the UST from which the sample was collected. Fill port samples from UST-1 through
UST-6 were collected with a decontaminated hand auger from a depth of about two ft-bgs. Fill port samples
from UST-7 and UST-8 were collected from the surface, as the manholes surrounding these fill ports were
cemented in place.
All soil samples were submitted to Pace Analytical Services (Pace), an analytical laboratory certified in North
Carolina and located in Asheville, North Carolina, for analysis. Samples were submitted for Gasoline Range
Organic Total Petroleum Hydrocarbons (TPH-GRO) analysis by EPA Method 8015B, if the former contents of
the associated UST were gasoline-related. For USTs that contained diesel or kerosene, the soil samples
were analyzed for TPH-GRO and TPH-Diesel Range Organics (DRO) by EPA Method 8015B. According to
observations made of the free product and review of DENR records, UST-1, UST-2, and UST-4 contained
gasoline. Therefore, soil sample collected from the fill ports and the border of these USTs were analyzed for
only TPH-GRO.
UST soil sample locations are displayed on Figure 3. A summary of soil sample depths, associated USTs,
and analytical results are included as Table 3.

C, respectively. The DENR Guidelines (DENR 2008a) did not require soil lithology to be described for the
MW-1 and MW-3 borings (the “non-source area” well borings).
All equipment, well materials, and tools that entered the borehole were steam-cleaned with a high
temperature pressure washer, prior to installation. The monitoring wells were composed of two-inch nominal
inside-diameter (ID) Schedule 40 PVC. The 0.010-inch machine-slotted well screens used for the monitoring
wells were ten feet in length for MW-2 and MW-3, and 15 feet in length for MW-1. A clean, washed, well-
rounded silica sand was installed to two feet above the top of the well screen to function as a filter-pack
between the well screen and the formation. A two-foot bentonite seal was installed on top of the sand pack
and hydrated, followed by cement grout mixture installed to ground surface.
Each monitoring well was designed so that the well screen would bracket the water table. The depth to the
well bottom for MW-1, MW-2, and MW-3 was 58, 53, and 53 ft-bgs, respectively. Monitoring well
construction details are included in Table 2 and Appendix C.
Consistent with the Guidelines for Assessment and Corrective Action for UST Releases (DENR 2008a),
Altamont collected soil samples from the monitoring well installed in the “source area” (MW-2), at ten-foot
intervals. Specifically, soil samples were collected at 8-10 ft-bgs, 18-20 ft-bgs, 28-30 ft-bgs, 38-40 ft-bgs,
and a sample collected just above the water table at 43-45 ft-bgs. The DENR Guidelines did not require soil
samples to be collected from the MW-1 and MW-3 soil borings (the “non-source area” well borings).
The organic vapor analyzer (OVA) with photo-ionization detector (PID) used on-site stopped working on March
22, 2011, due to an expired bulb. A functioning PID had to be shipped to the site the next day. Therefore,
the soil samples collected from the boring for MW-2 were placed in plastic Ziploc® bags and stored on ice
overnight. The following day, March 23, 2011, each soil sample was split, and one of the split samples was
allowed to volatilize in a warm environment for at least 10 minutes. The volatized samples were then
screened with the PID using proper headspace screening techniques (EPA 2001), as described in the BSA
Work Plan (Altamont 2011b) to help Altamont personnel preliminarily identify whether the soils were
potentially contaminated. The headspace screening results are recorded on the boring log for monitoring
well MW-2 (Appendix B).
The iced samples that were the counterparts to the volatilized samples were submitted to Pace for analysis
of volatile organic compounds (VOCs) by EPA Method 8260B, semivolatile organic compounds (SVOCs) EPA
Method 8270D, Massachusetts Department of Environmental Protection (MADEP) volatile petroleum
hydrocarbons (VPH), and MADEP extractable petroleum hydrocarbons (EPH), in accordance with DENR
guidance (2008a).
After installation of the monitoring wells and a minimum 24-hour period for grout curing, each monitoring
well was developed using a Mini-Monsoon submersible pump. To enhance the removal of sediment, the
pump was intermittently agitated by rapid short upward strokes from the bottom of the well. During
development, the field parameters of pH, temperature, dissolved oxygen (DO), specific conductivity (SC), and
oxidation-reduction potential (ORP) were measured approximately every five minutes with a calibrated YSI
multi-parameter water quality meter. Turbidity was also measured every five minutes with a calibrated
Lamotte 2020 turbidity meter. Equipment calibration information is included in Appendix D. The monitoring
wells were pumped until these parameters stabilized. Parameters were considered stabilized if, after three
consecutive readings:
• pH ± 0.1 unit
• SC ± 3%

Monitoring well top-of-casing (TOC) elevations were measured by Altamont personal using laser level
surveying equipment. Altamont personnel are not North Carolina licensed surveyors. Therefore, monitoring
well elevation data are considered to be approximate and not meant to substitute for a survey.
All of the monitoring wells were gauged using a decontaminated Solinst water level meter prior to well
development on March 23, 2011, and prior to groundwater sampling on March 24, 2011. Monitoring well
elevation data and water level gauging data are included on Table 2.

lack of precision in the laboratory instruments. Altamont did not detect any quality control problems with the
laboratory analysis. In accordance with the Site-Specific QAPP, a field audit was conducted by Stuart Ryman,
P.G. of Altamont on March 23, 2011. The field audit was conducted to ensure that the proper protocols
were being followed during field work. No deficiencies were noted.

Concentrations of TPH-GRO exceeding the UST Action Level of 10 mg/kg were detected in soil samples
collected from the UST fill ports of UST-1, UST-2, and UST-7. Concentrations of TPH-DRO exceeding the UST
Action Level of 10 mg/kg were detected in soil samples collected from the fill ports of UST-3, UST-7, and
UST-8. The most elevated concentration of TPH-GRO detected in a fill port sample was detected in sample
SB-FP-1 (1,960 mg/kg). The most elevated concentration of TPH-DRO detected in a fill-port sample was
detected in sample SB-FP-8 (41,300 mg/kg). These results suggest that overfilling likely occurred at UST-1,
UST-2, UST-3, UST-7, and UST-8 in the past. Analytical results are included in Table 3.
Soil sample analytical results collected from the sides of the USTs at just below the tank bottom displayed
concentrations of TPH-GRO above the UST Action Level of 10 mg/kg near only UST-6. Concentrations of
TPH-DRO were detected above the UST Action Level of 10 mg/kg near UST-3, UST-6, UST-7, and UST-8. The
most elevated concentration of TPH-GRO collected from the sides of the USTs was detected in SB-14 (2,930
mg/kg), which was collected from the west side of UST-6. The most elevated concentration of TPH-DRO
collected from the sides of the USTs was detected in SB-31 (2,360 mg/kg), which was collected from the
east side of UST-8. These data indicate that petroleum constituents have impacted soil near the south side
of UST-3, near the west side of UST-6, the north side of UST-7, and the east side of UST-8. Sample locations
are displayed on Figure 3, and analytical results are included in Table 3.
None of the analytical results from soil samples collected from below product lines contained concentrations
of TPH-GRO or TPH-DRO above UST Action Levels. Sample locations are displayed on Figure 3, and
analytical results are included in Table 3.
No VOCs, SVOCs, or VPH hydrocarbon fractions were detected in excess of the Soil-to-GW MSCCs from the
soil samples collected at 10-foot intervals during the installation of monitoring well MW-2. Analytical results
are included in Table 4.

Site-wide gauging of monitoring wells occurred just before well development on March 23, 2011, and just
before monitoring well sampling on March 24, 2011. The depth-to-groundwater data from these two events
were within 0.02 ft of respective measurements for each monitoring well. The depth-to-water btoc in
monitoring wells MW-1, MW-2, and MW-3 was measured at 45.76 ft, 46.61 ft, and 45.97 ft, respectively.
Relative groundwater elevations for monitoring wells MW-1, MW-2, and MW-3 were calculated to be 7.43 ft,
7.96 ft, and 9.50 ft, respectively (Table 2). These elevations were plotted on site map and used to calculate
groundwater flow direction (Figure 4). Apparent groundwater flow direction is to the southwest.
Analytical results from groundwater samples collected from monitoring wells MW-2 and MW-3 did not
contain any SVOCs or VOCs in exceedance of DENR NC GWQS. Analytical results from the sample collected
from monitoring well MW-1 contained no SVOCs, but did contain multiple VOCs at concentrations above the
NC GWQS. Specifically, 1,2-dichloroethane was detected at a concentration of 17.7 micrograms per liter
(µg/L), above the NC GWQS (0.38 µg/L). 1,2-dichloropropane was detected at a concentration of 1.5 µg/L,
above the NC GWQS (0.51 µg/L). Benzene was detected at a concentration of 2.3 µg/L, above the NC
GWQS (1 µg/L). Diisopropyl ether was detected at a concentration of 195 µg/L, above the NC GWQS (70
µg/L). These concentrations were also compared against the Gross Contaminant Level (GCL) for GW and
were found to be below those action levels. Analytical results are summarized in Table 5.

Definitions of Sources
Tank: means the tank that stores the product and is part of the underground storage tank system
Piping: means the piping and connectors running from the tank or submersible turbine pump to the dispenser or other end-use equipment (Vent, vapor recovery, or fill
lines are excluded.)
Dispenser: includes the dispenser and the equipment used to connect the dispenser to the piping (e.g., a release from a suction pump or from components located above
the shear valve)
Submersible Turbine Pump (STP) Area includes the submersible turbine pump head (typically located in the tank sump), the line leak detector, and the piping that
connects the submersible turbine pump to the tank
Delivery Problem: identifies releases that occurred during product delivery to the tank. (Typical causes associated with this source are spills and overfills.)
Other: serves as the option to use when the release source is known but does not fit into one of the preceding categories (e.g., for releases from vent lines, vapor
recovery lines, and fill lines)
Unknown: identifies releases for which the source has not been determined
