1. PURPOSE
This procedure provides guidance on how to work at heights safely in carrying out work.
2. SCOPE
2.1. Provide a safe, hygienic and healthy workplace.
2.2. To standardize a uniform procedure for issuing work permits at heights within PT. IVSSI
2.3. To provide protection for personnel and property in order to maintain safety against the risk of
falling if the employee is working at a height of at least 1.8 meters or more, as well as fall protection
on dangerous equipment.
2.4. Develop and implement procedures to reduce or eliminate the risk of falls or falls from workplaces
at height and protect employees or contractors from being crushed by falling objects.
3. DEFINITION
3.1. Working at height is all work/projects where employees work at a height of 1.8 meters or more,
above a lower level for example; work on roofs, building frameworks and others.
3.2. The fall protection system is the use of safety equipment components such as body safety harnesses,
shock-resistant straps, deceleration equipment, vertical lifelines and tethers that are interconnected
to stop the risk of falling.
3.3. Subcontractors are other parties who carry out work within PT. IVSSI both individually and in
groups.
3.4. The HSE Committee is the Safety, Occupational Health and Environment Advisory Committee
4. RESPONSIBILITY
The HSE Departement is obliged to supervise the implementation of this procedure carefully and in
depth.
5. PROCEDURE
5.1. Risk Assessment
a. Identification of work or objects at risk of falling
b. Evaluation of the risks associated with working at heights
c. Identification and implementation of control measures to reduce the risk of falls

5.2. Requirements for working at Height


a. Employees working at a height of more than 1.8 meters must be equipped with appropriate
Personal Protective Equipment (PPE), such as: Full body hardness, Helmet, adequate ladders etc.
The relevant supervisor must issue a work permit at height and supervise the safety of his crew.
b. Full body harness is required for any height of 1.8 meters (6 feet) or more.
c. Fixed/permanent ladders over 2.1 meters (7 ft) shall be provided with a safety ring built around
the ladder at a height of 2.1 m (7 ft).

d. Gaps in Floors and Walls:
● Locations where a person could fall from a distance of more than 1.2 m (4 feet) must be
secured using standard guardrails and side guards (standard guardrails consist of top and
middle support beams, and posts) on all exposed sides except if there is access to ramps,
steps, or permanent stairs.
● If there is a potential hazard of materials or equipment falling through gaps in floors or
walls, these gaps must be protected with side guards or closing partitions.
5.3. Procedures for working at height
a. Department/contractor who will carry out activities at a height of 1.8 meters or more must issue a
work permit.
b. Before the permit to work at height is issued, the Safety Officer will identify the location to be
worked on and inspect the protective equipment for working at height before use.
c. Permit to work at height is only valid for one day.
d. Protective equipment for working at heights must be used when working after having previously
been declared fit for use by the safety officer and limiting access where there is a risk of falling
hazard or falling material.

e. After all requirements for working safely at height have been met by the Department/contractor
who will carry out work activities at height, a permit to work at height will be issued by the
safety officer and signed by the work location supervisor.
f. After completing the work at a height, the original copy of the permit is returned to the PIC at the
work site with a note regarding the completion of the work, such as: "It was completed according
to the time period given and without any accidents".
5.4. Inspections and Audits
a. The Safety Officer regularly checks Personal Protective Equipment for Working at Heights and
supervises work areas at heights.
