    Main engine oil-water leaking

    ANALYSIS OF CAUSE OF DEFICIENCY

    Lack of maintenance and inspection regime as per PMS on board. The Main engine unit no.1 was notice that cooling
    water jacket leakage.
    According to previous record once overhauled of main engine the O-ring was inspected and condition was
    satisfactory then used the same O-ring when installed back of main engine liner cooling water jacket.

    CORRECTIVE ACTION PLAN

    1.Regulary maintenance and inspection to be carried out and ensure equipment are in good order.
    2.Main engine jacket cooling water temperature to be maintained as per manufacture guideline.
    3.Plan maintenance to be carried out as per schedule (PMS).
    4.Chief engineer to be ensure to corrects procedure and proper maintenance of main engine with inspection and
    review during maintenance.
    5.Ensure the spare part are in corrects part number as per manufacture maintenance booklet.
    6.To ensure spare part maintained and available on board .

    IMMEDIATE CORRECTIVE ACTIONS (if required, attach supporting docs/pics/ Training Record)

    Main engine unit no.1, cylinder cover and liner cooling water jacket had been open up for inspection and found O-ring
    poor condition and hardened that been used since long time.
