This letter is sent to employees to notify them that they have been released to return to
work. The letter includes all information necessary to defend a termination petition should
the employee not return to work as ordered by this letter. If the employee voluntarily
returns to work, this letter is not required.
Dear [EMPLOYEE]:
You are, therefore, ordered to return to work on [DATE] and report to [LOCATION],
[SUPERVISOR NAME] at [TIME]. Your salary effective [RETURN TO WORK DATE] is $[RATE].
Failure to report to this assignment could jeopardize your workers’ compensation indemnity
benefits. In addition, you could receive discipline up to and including termination if you do
not return to work.

I hope this notice finds you safe and well during this unprecedented time. While we
have all been working remotely over the past few months, we are pleased to announce
[Company name] has begun the process of reopening our buildings and returning
employees to the workplace. This process will be implemented in three phases as
follows:
Phase one includes [insert categories, classifications, departments or other groups],
who will return to the office on [date].
[Optional text for employers that will give employees the choice whether to return to the
office or continue working remotely.]
If you do not wish to return to the office on your scheduled date, you have the option to
continue working remotely through [date]. Please speak with your manager regarding
your plans no later than [date].
Safety is of the utmost importance during this return-to-office process, and we have
made numerous changes to our policies and procedures, including the following [edit

All employees entering our buildings will be screened for COVID-19 symptoms
and possible exposure.
No visitors will be permitted to enter [Company name] buildings.
Common areas and frequently touched surfaces are being cleaned daily.
Business hours have been modified to allow for staggered work shifts to reduce
the number of people in the building at one time.
Workspace layouts and seating arrangements are revised to allow for social
distancing.
Meeting rooms, break rooms and other communal areas have reduced seating
and capacity limits.
Business travel remains restricted to essential travel only.
