General Guidelines
1. All level II students enrolled in the current semester are required to comply with the
stipulated requirements for each of their missed rotation due to the Enhanced Community
Quarantine (ECQ) brought about by the COVID-19 pandemic to earn their numerical grade
in their RLE.
3. Students are given the choice to decide whether to have their outputs encoded or
handwritten, whichever is more convenient for them.
4. For students who opt to have their outputs encoded, below are instructions to be followed:
a. Students should convert their files into PDF for submission unless otherwise
specified by their Clinical Instructors (C.I.s).
b. The platform for submission is Google Mail, whereby, respective
C.I.s shall provide their email address to their students for purposes of submission
and correspondence.
c. For the subject title of the email, students should follow the format:
*Rotation Area* Requirements (for example: ZCMC Ward 1 Requirements)
d. Students should indicate their complete name, section, and rotation dates in the
body of the email.
e. Students should remember to follow online netiquettes when sending an email.
f. Students can expect their respective C.I.s to provide them with acknowledgement
receipt to confirm that their emailed outputs had been received in full without
technical issues.
g. Students are encouraged to print their submitted outputs upon their return to school
for documentation purposes and for their future consumption.
5. For students who opt to have their outputs handwritten, below are instructions to be
followed:
a. All outputs shall be written in their RLE notebook for the current semester.
b. For students whose RLE notebooks were not returned prior to the implementation
of the community quarantine, other available notebooks of the same size as their
RLE notebooks can be used as an alternative.
c. For students without available alternative notebooks, bond papers can be used as
substitute.
d. Students can submit their handwritten outputs online by either taking a photo or
scanning their outputs and send them to their respective C.I.s via Gmail.
6. For students who opt to submit their outputs in-person, updates will be provided
regarding instructions for submission.
2. For the Case Study, below are the instructions for compliance:
a. Students should follow the Study of Illness Condition (SIC) Format provided by
the level II faculty (see table 1 below).
b. For the illustration of organ system involved, students can either draw their
illustrations or source pictures from the internet.
c. Students should comply with the specific number of case studies and further
instructions stipulated by their respective C.I.s.

5. For the Health Teaching Plan (HTP), below are instructions for compliance:
a. Students should follow the HTP Format provided by the level II faculty (see figure
2 below). However, the evaluation component is to be omitted.
b. Students should comply with the specific number of HTPs and further instructions
stipulated by their respective C.I.s.
6. For students who opt to have their NCP, HTP, case study, and drug study on a bond paper,
they should write it in landscape orientation to maximize the use of space and for ease of
checking on the part of their C.I.s. For the case presentation, portrait orientation will do.
7. For every first page of their requirements per area, students should start with a header
containing the following information:

8. Students do not need to include the Terminal Performance Objective, Enabling Objectives,
and Planned Table of Activities.
9. For questions and clarifications regarding their requirements, students are encouraged to
have a contact person per rotation who can forward their concerns to their respective C.I.s.

• The three (3) NCPs correspond to the following problems: 1 physiological,
