We present the clinical case of a man of 68 years who was admitted for dizziness and sensation of loss of conscience. The clinical examination revealed a body temperature of 37.5 degrees C and a murmur of mitral regurgitation. The echocardiogram showed a severe mitral regurgitation and left cavitie's dilatation; transesophageal echocardiogram showed a vegetation in the anterior leaflet of the mitral valve. In blood cultures grew a Gram-negative bacteria identified as Bartonella spp. A PCR demonstrated that it was a Bartonella quintana. The patient was treated with gentamicin, doxiciclin and ceftriaxone with satisfactory evolution. The remaining mitral insufficiency awaits surgical treatment.

Notably, the number of damaged crop species substantially varied among the prefectures, possibly due to differences in terms of the extent of countermeasures against crop raiding by macaques. To discuss the regional variation in the preference for specific crop species, quantitative and qualitative data should be standardized among the prefectures. The information on the crops damaged by macaques is generally retrieved from government reports, which are only retained for a short period; therefore, digital archiving is necessary for their future use. In addition, we discuss future challenges about the use of information on the diet of crop-raiding macaques. Moreover, we emphasize the importance of collaboration between researchers of fundamental and applied research.

We conducted laboratory experiments to assess the disease control effects of pH-decreasing fertilizer as a new control material for lettuce big-vein disease. Results show that the occurrence of big-vein disease was suppressed in concomitance with the descent of soil pH by applying the fertilizer.

Pyoderma gangrenosum (PG) is an uncommon rapid progressive and ulcerating skin disease of unknown etiology. The early lesion shows a clinical feature similar to postoperative infection, so the diagnosis of PG is difficult. In this report, we present a rare case of PG arising after surgery for oral cancer. A 72-year-old woman received radical resection for lower gingival cancer with reconstruction using delto-pectoral flap (D-P flap). Six days after the surgery, she had pyrexia and water diarrhea. The submandibular skin around the insertion site of the D-P flap became reddish with diffuse swelling, pus discharge and severe tenderness. A provisional diagnosis was postoperative infection of the wound. Although the patient was given repeated courses of antibiotics, debridement and drainage, the lesion extended to the whole neck of the affected side and a necrotic ulcer with bluishpurple undermined borders surrounded by erythema was observed 13 days after the surgery. The patient was referred to a dermatologist and PG was strongly suspected. From the postoperative 15 th day, treatment with prednisolone and minocycline over the next several days improved her symptoms dramatically. Histopathological examination revealed that severe inflammatory cells, predominantly of neutrophils, had infiltrated into the dermis without bacterial components. These clinical and histopathological findings fulfilled the diagnostic criteria of PG. PG should be taken into consideration as a potential complication in oral and maxillofacial surgery too.

This research investigates the technique for cherry trees maintenance in the early 20th century by literature, especially articles of "Sakura-Japanese Cherry Blossoms (Journal)" (1918-1943) which was published by Society of Cherry Tress. As a result, the total number of articles was 444, and these included 47 articles about technique for cherry tree maintenance. These 47 articles mainly revealed the following 5 categories: "Pruning", "Fertilizing", "Planting", "Conservation", and "Pests Management". In addition, the contents of descriptions in 47 articles were fundamentally equivalent to the present recognition. However, some technique had been evolving from the early 20th century to present with diversifications of machinery and materials. The results and considerations in this research are important for keep improving the technique of cherry trees maintenance in the future. English abstract into Japanese. A 70-year-old man with a medical history of diabetes mellitus presented to a local clinic with seven days history of right flank pain and fever. Because there was no symptomatic improvement after one-week antibiotic administration, abdominal ultrasonography and computed tomographic scans were performed to show mass lesions in the right hydropelvis, and he was transferred to our hospital. Retrograde pyelography revealed a retrocaval ureter and a ureteral stent was indwelt. After the symptoms improved, the lesions were removed by percutaneous nephrostomy and fungal balls were diagnosed as Candida parapsilosis after culture. After intermittent one-week irrigation of the renal pelvis with normal saline, the nephrostomy tube was removed. In six-month follow-up, mild hydronephrosis remains without fungal ball recurrence. To the best our knowledge, there has been no case report of Candida parapsilosis fungal balls in the urinary tract.

Postoperative saliva leakage associated with wound dehiscence is one of the serious postoperative complications of major head and neck oncological surgery. Usually, a pharyngocutaneous fistula has to be formed for appropriate drainage of the infectious saliva to protect cervical vital structures, including the carotid artery. After the microvascular circulation around the fistula becomes stable, the fistula can be closed safely using a hinge flap and local rotation or a free island flap. Conventionally, the pharyngocutaneous fistula is maintained by plugging it with ointment gauze and covering it tightly with dry gauze and adhesive tapes ; the tracheal stoma has to be managed with a cuffed cannula to prevent aspiration of saliva.
