This study investigated the utility of confirmatory factor analysis (CFA) and item response theor... more This study investigated the utility of confirmatory factor analysis (CFA) and item response theory (IRT) models for testing the comparability of psychological measurements. Both procedures were used to investigate whether mood ratings collected in Minnesota and China were comparable. Several issues were addressed. The first issue was that of establishing a common measurement scale across groups, which involves full or partial measurement invariance of trait indicators. It is shown that using CFA or IRT models, test items that function differentially as trait indicators across groups need not interfere with comparing examinees on the same trait dimension. Second, the issue of model fit was addressed. It is proposed that person-fit statistics be used to judge the practical fit of IRT models. Finally, topics for future research are suggested.

In this article we present a model for the interaction between physical, psychological and social... more In this article we present a model for the interaction between physical, psychological and social factors in the production of subjective somatic complaints, which is the most common reason for visiting a doctor. Especially if the patient has subjective somatic symptoms for which there is no apparent organic reason it may be important to motivate the patient to examine all the different factors that contribute to the vicious circle he/she often enters into. The following elements of this circle are explained: Physical (symptoms, signs, findings), beliefs (thoughts, cognitive processes and cognitive schemas), feelings (emotions), behaviour (all we say and do) and the so-called X-factor, which represents everything we do not know. The model might help us in our relationship to the patients, and can be used both in connection with known somatic disease and with conditions with a multifactorial genesis, like somatoform disorders.

A total of 152 patients with recurrent peptic ulcer disease were treated with bismuth subnitrate,... more A total of 152 patients with recurrent peptic ulcer disease were treated with bismuth subnitrate, oxytetracycline and metronidazole. The 51 patients in Group 1 were included following a study of the effect of cognitive psychotherapy on the recurrence rate after curative treatment for duodenal ulcer (DU). Group 2 comprised 101 consecutive patients referred to us with recurrent peptic ulcer. Their Helicobacter pylori (Hp) status was assessed before, and 4 weeks after, discontinuation of treatment, using the 14C-urea breath test. The primary aim of the study was to determine the effect of the triple treatment regimen on Hp eradication. All but two patients were Hp-positive prior to treatment. Four weeks after termination of treatment, Hp was found to have been eradicated in 50 out of 51 patients (98%) in Group 1, and in 86 out of 97 patients (89%) in Group 2. The respective ulcer healing rates for the two groups were 100% and 95%. Fifty percent of the patients noted moderate to severe adverse effects, mainly nausea and diarrhea, but only 5 patients discontinued treatment prematurely. The results indicate that virtually all patients with recurrent peptic ulcer disease have gastric Hp infection, and that the present triple treatment is highly effective in terms of both Hp eradication and ulcer healing.

Eradication of H. pylori cures peptic ulcer disease. Bismuth and antibiotics in various combinati... more Eradication of H. pylori cures peptic ulcer disease. Bismuth and antibiotics in various combinations were tested at a single university hospital. The standard treatment was bismuth subnitrate, metronidazole and oxytetracycline. A positive biopsy urease test was the main inclusion criterium, whereas a negative 14C-urea breath test performed at least four weeks after cessation of therapy was the main indicator of cured infection, and performed later for reinfection. In separate studies we tested the absorption of bismuth subnitrate compared to other bismuth preparations, what aspects of bismuth-based triple therapy are most important to obtain maximum eradication, what can be done to decrease side effects, and the reliability of diagnostic methods. The absorption of bismuth from bismuth subnitrate was very low. More than 90% cure rates were usually obtained.
