This case report describes the treatment of a 26-year-old man with an anterior open bite using 2-jaw orthognathic surgery. He had a class III molar relationship, open bite, crossbite, and retroclined lower incisors. Treatment involved Lefort I maxillary advancement with expansion and down fracture to correct the open bite and midface deficiency. Bilateral sagittal split osteotomies corrected the open bite and mandibular asymmetry. The treatment resulted in correction of the facial and dental aesthetics with a class I occlusion and improved overbite and overjet.

“A 26 years old White man visits the
orthodontic consultation with a chief complaint of open bite . He has previously been an orthodontic patient when he was 13 years old . Now diagnosed by a speech pathologist who observed that he had severe articulation impairment and a tongue thrust habit ‘’ CLINICAL FEATURES EXTRA ORAL FEATURES
• FACE FORM : Mesoproscopic
• PROFILE : Straight • NASOLABIAL ANGLE : obtuse • Nose deviated to the right • Chin deviated to the Left • Maxillary hypoplasia and mild mandibular prognathism INTRA ORAL FEATURES • MOLAR RELATIONSHIP: class III relationship bilaterally • CANINE RELATIONSHIP : class lll on right and class 1 relationship on left • MAXILLARY MIDLINE : 1mm right to the facial midline • MANDIBULAR MIDLINE :2mm left to the facial midline • OVERJET : -1.0 mm • OPEN BITE : 7.0 mm on the maxillary right central incisor with lateral open bites • CURVE OF SPEE : flat ( contact only on second molar ) • MINIMAL GINGIVAL ATTACHMENT IN THE MANDIBULAR ANTERIOR • ANTERIOR AND POSTERIOR TEETH CROSSBITE • GINGIVAL RECESSION ON THE MAXILLARY FIRST MOLAR PANORAMIC RADIOGRAPH shows that all third molars have been extracted BOLTON DISCREPENCY observed due to small maxillary lateral incisor CEPHALOMETRIC ANALYSIS Lateral cephalometric analysis indicates • Class III pattern • Hyperdivergent growth pattern • Maxillary incisor has normal inclination • Mandibular incisors are retroclined TREATMENT OBJECTIVES Correction of jaw deformities Correction of anterior Open bite Correction of Bilateral crossbite Correction of midline Establishing class I molar and canine relationship Obtaining normal overbite and overjet Improvement of facial and dental esthetics TREATMENT ALTERNATIVES 1. Combination of orthodontic and orthognathic surgery • Lefort 1 maxillary advancement osteotomy with expansion of the posterior segment (differential impaction ) • Down fracture of the anterior segment to correct open bite, transverse discrepancy, midface deficiency ,occlusal cant ,maxillary midline and maxillary incisor display • Bilateral sagittal split osteotomy will correct the open bite , mandibular asymmetry and midline.
