Nursing Responsibilities of

the Nurse in Relation to the

Mechanisms of Labor

Ryan Robert V. Ventolero

Topic Description:
This topic deals with the nurse’s responsibilities with
regards to the mechanisms of labor in normal cephalic
delivery.

Central Objectives:
Define and understand mechanisms of labor and the
cardinal movements, and the second stage of labor
and know the nursing responsibilities during this stage.
Identifying Information

Mechanisms of Labor
Also called the cardinal movements, is“the series of
movements that occur on the head in the process of
adaptation during its journey through the pelvis.”
(Birat Health College, 2019)
Second Stage of Labor
“it begins when the cervix is fully dilated, and the
woman feels the urge to expel the baby.”
(Nidhin, 2019)
Anatomy
Descent
“is the downward movement of the
biparietal diameter of the fetal head to
within the pelvic inlet” (Foley &
Pillitteri, 2010)

The Mechanisms Of Labor

Flexion
“fetal head comes into contact with the
pelvic floor, cervical flexion occurs”
(Geeky Medics, 2018)

The Mechanisms Of Labor

Internal Rotation
“fetal head rotates from a left or right-
occipito-transverse position to an
occipito-anterior position” (Geeky
Medics, 2018)

The Mechanisms Of Labor

Extension
“occiput slips beneath the suprapubic
arch as the head extends and the nape is
pivoting against the arch” (Geeky
Medics, 2018)

The Mechanisms Of Labor

External Rotation
“immediately after the fetal head is born,
the head then rotates back to the
diagonal or transverse position of the
early part of labor” (Foley & Pillitteri,
2010)

The Mechanisms Of Labor

Expulsion
“when the shoulders are born, the rest of
the baby is born easily and smoothly
because of its smaller size.”

Clinical Scenario
A 26-year-old primigravida is admitted at the
birthing room. Her husband is an OFW and they
have been married for 1 year. She has no family
member with her as of the moment. Patient’s VS are
stable
Gynecological history
Had regular menstrual cycles since menarche. 1st,
2nd, and 3rd trimesters with no any abnormal
complains and normal ultrasonic scans. No Pap
smear done, and contraception never taken.
