SCOPE This procedure shall apply to all staffs and subcontractor who involved in any routine or nonroutine works that potentially exposed to hazard.

PROCEDURE Type of Work required Permit to Work (PTW) 6.1.1 The application of PTW shall be but not limited to the following works/ activities: i) ii) iii) iv) v) vi) vii) Hot Work - in close area or near to inflammable / combustible materials Confined space Cleaning of water tank, working under the ground (manhole). Working at height above 3 meters and/ or requires the erection of scaffolding External glass / building cleaning, changing fluorescent tube, etc. Operating of gondola, personal or good lifter or other type of vertical lifters. Major repair of lift car. Energize electrical that required the implementation of Lock Out /Tag Out (LOTO) procedure Use or handling the chemical hazardous material

viii) Any hazardous activity identified in HIRADC list, which not listed above. 6.1.2 The application of PTW is limited to employees and vendors under OSI control unless approved by building owner. 6.2 Issuance, Approval and Safekeeping of Work Permit 6.2.2 6.2.3 6.2.4 Permit To Work form shall be filled-up by the relevant staff or vendors who required to perform any work listed above. PTW form must outline the basic SHE requirements and the form must be properly controlled according to company procedure. Briefing on potential hazards and safety precautions shall be carried out by the HSE Representative/ HOS prior commencement of work. The PTW issuer shall not be the same person to supervise the work. The completed PTW shall be made available at the time of entry to all authorised entrants, by posting it at the entry portal. The PTW shall be valid for 1 days for each task carried out by site staffs or sub-contractors. The new Permit to Work shall be issued on the 2nd day if the same task still on going. Supervisor / SHE Representative / appointed staff shall conduct random inspection from time to time during work in progress. Staff shall sign off and indicate the time of random inspection in the PTW form (column provided). a) The operation covered by the entry Permit to Work have been completed; or b) A condition or scenario that does not cover under the Permit to Work due to significant changes in risk arises in or near the hazardous task.
