    All participants and students must complete this form before participating in any activities
    related to the Saint Louis University Lantern Parade.
    If the participant is under 18 years old, a parent or guardian must also sign this form.

    From the foregoing, I recognize and understand the risks of physical injury
    inherent to trainings, rehearsals, and event proper performances and I personally
    and fully assume those risks and take full responsibility therefor. I hereby release
    Saint Louis University, the event sponsors, the employees, and the committee of
    the annual Saint Louis University Lantern Parade from any and all liabilities for
    injuries sustained or illnesses contracted while attending or participating in the
    trainings, rehearsals, or event proper itself.

    I further state that there are no health-related reasons or problems which


    may restrict my or my child’s participation in this activity.
