I understand that participation in the activity involves a certain degree of risk. I have carefully considered the risk
involved and have given consent for my son/daughter to participate in the activity. I understand that participation in the
activity is entirely voluntary and requires participants to abide by applicable rules and standards of conduct.
Upon proper dissemination of school policies, rules and regulations to ensure the utmost safety of all students, during
and after the activity but my son/daughter fails to abide, I release the Colegio de San Gabriel Arcangel and its officers,
faculty and staff, the activity coordinators, and all volunteers, related parties, or other organizations associated with the
activity from any and all claims or liability arising out of this participation (Nauunawaan ko na ang pakikilahok sa
aktibidad na ito ay may kaakibat na panganib. Maingat naming isinasaalang-alang ang panganib na dulot nito
at kami ay magbibigay ng pahintulot para sa aming anak na lumahok sa aktibidad. Nauunawaan namin na ang
pakikilahok sa aktibidad na ito ay kusang-loob at kailangan ng mga kalahok na sumunod sa mga naaangkop na
patakaran at pamantayan ng pagsasanay. Pinapalaya ko ang Colegio de San Gabriel Arcangel at ang mga
opisyal, guro at kawani, coordinator ng aktibidad, at lahat ng mga volunteer, mga kaugnay na partido, o iba
pang mga organisasyon na nauugnay sa aktibidad na ito ay walang pananagutan na nagmumula sa
pakikilahok na ito).
It is understood that the school cannot be held liable for any injuries that possibly occur while safety precautions have
been given and are administered (Nauunawaan na ang paaralan ay hindi mananagot para sa anumang mga pinsala na
maaaring mangyari habang ang mga pag-iingat sa kaligtasan ay ibinigay at pinangangasiwaan).

I have read the above ACTIVITY CONSENT/WAIVER FORM and am fully familiar with the contents thereof. I give permission for the
student named above to participate in the activities of this institution, including any special events/activities described above. In
consideration for allowing the participation of the student in these activities, I hereby consent to the ACTIVITY CONSENT/WAIVER
FORM, including the Release of Liability above, on behalf of the student and agree that this ACTIVITY CONSENT/WAIVER FORM
shall be binding upon me, my family, heirs, legal representatives, successors, and assigns.

RULES BEFORE, DURING AND AFTER THE HOLY RETREAT

1. Students should be at the assembly area (CDSGA Ground Floor) at 2:00 AM and the departure time is 4:00 AM. Let us observe
AMERICAN TIME.
2. First come first served basis should be strictly followed in occupying a seat in the bus you are listed.
Note: ( No one is allowed to transfer to another bus stick to your assigned bus number)
3. Students should bring their EXTRA BAON on the way to the place and in the 2nd day in going home.
4. Everyone should see to it that no one of his/her classmate is left behind.
5. Attendance should be checked before leaving the area.
6. Students should bring personal hygiene kit. (e.g. toothbrush, soap, shampoo, towel)
7. Students are not allowed to visit a relative during and after the retreat.
8. Students should be responsible of their own belongings. Lost things are his/her own responsibility.
9. Late comers are not allowed to join (No Refund Policy)

Code of Behavior during the RETREAT

1. Students should demonstrate good discipline before, during and after the retreat.
2. Bringing and drinking alcoholic beverages is strictly prohibited before, during and after the retreat.
Any student found to be under the spirit of liquor or prohibited drugs will be BANNED FROM JOINING THE RETREAT and may
NOT BE ALLOWED TO GRADUATE.
3. Bringing Cigarettes/Smoking is strictly prohibited before, during and after the retreat.
4. Loitering is not allowed while session is ongoing.
5. Students should wear PE or Department Uniform and ID going to the Retreat House. They should wear their proper attire during
and after the Retreat. Anyone who is not in proper attire will not be allowed to enter the holy retreat room. It is advised however
that extra clothes be brought along. (No wearing of sando, shorts, sleeveless and slipper)
6. Wearing of shorts/ sando / slipper is allowed during output base activities.
7. Students should strictly follow the rules given by the retreat master,
8. VANDALISM and STEALING of any sort of are strictly prohibited and are criminal acts.
9. FALLING IN LINE is necessary in going to the dining place. FOOD IS buffet. TAKE ONLY an amount of foods
that u can consume. No Left-over foods.
10. ALWAYS BE ON TIME in all of the planned activities.

THE MANAGEMENT

NOTE: Any violation of the above rule would mean will be subject for removal of school grant discount, disciplinary action and will not join the
graduation day.

 Student who will join with under medication/health concern should bring their medicine and notify the in-charge personnel.

 Parents/Guardian are required to attend to “SALUBONG PROGRAM” at CDSGA Activity hall
