compliance to the WSH Act and other relevant regulations.
Ensure our workplace premise is safe and without risk for everyone working at height in our workplace
premises.
Ensure the fall prevention plan in understood, implemented and maintained at all levels of the
organization.
We are also committed to continually improve our business practices to reduce its impact and effectively
manage the hazards associated with working at height.
In dealing with this project, we shall strive to achieve the following objective:
Work towards zero accident in particular chose related to falling form height.
Ensure that sufficient resources including manpower and training of competent person, which are essential to the
establishment, implementation and maintenance of the fall protection plan are made available for the worksite.

4) Hazard Identification and Risk Assessment
a. A risk assessment team was formed to conduct risk assessment for work activities at the work site including our
sub-contractors.

b. Establishment of an inventory to identify all main work activities with falling form height hazard for Risk
Management
c. Identify the hazards associated with workplace activities, assessed its risks levels and determine suitable control
measure are taken
d. Risk assessment is conducted and documented for main work activities / trade involving work at height including
risk control measures
e. Communicate the risk assessment to the workers involved in the work process
f. Implement the risk control measures as stated in the risk assessment

Elimination

This involves replacing the hazard by one that presents a lower risk. Mobile elevated work platform (MEWP) is
preferred of ladders for reaching high places.
The risk level is lowered as MEWP generally provides a higher level of fall prevention than ladders.
Engineering Controls
(Are there any physical means possible that limit falling form height hazard?)
Using of lanyard secured to all working tolls to prevent falling form height.
Refrain from leaving materials or storing of materials near to building edge/opening area
Provision of building edge lifeline with PE design endorsement to prevent worker falling from building
edge during installation and gondola operation.
Provide building edge barricade at work area where there is a likelihood of failing more than 3 metre
Provision of safety netting on the Gondola Cradle
Administrative Controls

5) Procedures
Our company had established communicated and implemented its safe work procedures and method statement
to all persons involved and affected by the WAH activities. This includes

Education through training on the proper use of PPE are briefed to workers through toolbox meetings or
safety trainings
All workers are required to put on appropriate PPE engaging in any work activity that may be hazardous
while working at height.
An inventory list of Fall Protection Equipment (Full body Harness) issued to workers had been established
Method statement of installation of anchorage and lifeline for use of safety harness or travel restraint belt
Definition
A full-body harness (FBH) is commonly known as a component of a body-holder device that connect a user into a
personal fall-arrest system (PFAS)
Fall arrest system that are used on site include wire rope lifeline installed at the building edge and vertical lifeline
installed for the usage by suspended scaffold (Gondola)
A full-body harness comprises the following components such as fittings, straps, ‘buckles or other elements that
support the body of a user, restrain the user during a fall and after the arrest of a fall.
Our company have established an inspection and maintenance regime to ensure that the workplace conditions and
any machinery, tools and equipment related to work at height are safe and without risk to workers and users in the
worksite
PPE will be checked for wear and tear, expiry etc. during daily toolbox meeting
Gondola weekly inspection (every 7 days) checklist by gondola contractor
Scaffold weekly inspection (every 7days) by Scaffold contractor
MEWP monthly service and maintenance record by machinery contractor
Lifting Platform weekly inspection (every 7days)by competent person
8) Training
Person working at height who are exposed to the risk of falling are provided with sufficient and adequate
information, instruction and training so that they are equipped with the skills and knowledge to perform their work
safely.
Is mandatory for MEWP operators to attend relevant MEWP Operator Training Course conducted by ATP approved
by Ministry of Manpower are deemed to be competent.
9) Incident Investigations

9.1 All accidents or incidents relating to fall from height will be investigated and analysed for the following
objectives:
a. Determine any underlying deficiencies in the existing fall protection system and plan which might have
contributed to the occurrence of the incident;
b. Identify the need for corrective action to the taken to address any such deficiencies as identified; and
C. Identify the opportunities for preventive actions to be taken to prevent any such incident form recurring
Our company shall promptly communicate to all relevant parties the results from any such incident investigated
(inclusive of lesson learnt and the corrective/preventive actions to be taken) shall be promptly communicated
A record of all accidents and incidents and their investigation findings and outcomes shall be taken shall be
properly documented.
The type of equipment required carrying out a rescue. Such equipment must be suitable for use in all-
foreseeable situations in the workplace. Existing equipment such as mobile elevated work platforms and
scissor lifts will be used for conducting rescue if possible
The installation of individual fall arrest systems and individual rope access systems should be at locations
where it is possible to assist or rescue a person quickly if required.
Ensure that all workers using arrest systems are familiar with devices before the commencement of work.
We have in place , an Emergency Preparedness Plan to respond to emergency situations such as worker suspended
at mid-height in Fall Arrest System and worker inside Gondola trapped in mid-height.
In the event of an emergency the emergency response team will be activated to carry out the rescue work as
according to the site condition.

Scaffold is a temporary structure with supporting platform which is used is used mainly for work,
access, egress or storage of materials during the work.
Every scaffolds should be designed, erected, maintained or be altered or be dismantled by
trained persons under the supervision of a competent person. Scaffold should also be
periodically (every 7 days or after rain) inspected by a competent person (scaffold supervisor)
All structures and appliances used as support for scaffolds, working platforms, gangways or runs
should be sound construction, have a firm footing.
Safe and secured means of access to and egress from the work area should be provided by using
ladders.
Working platform should be closely boarded. Every area of the working platform from which a
person is liable to fall form a distance of more than three (3)metres, should be provided with a
suitable guard-rail or guard rails of adequate strength to a height of at least one (1) meter above
the platform.
Toe-boards of at least 90mm height are to be installed. Every platform should be free from
unnecessary obstruction, material or rubbish, projecting nails and to avoid it from becoming
slippery.
All scaffold erector and user of all scaffolds shall use fall arrest equipment such as full body
harness and lanyards with proper anchorage points.
Adequate signage to be available to remind workers to wear safety harness before access any
scaffold.
C) A-frame ladder

Ladders should be of good construction, sound material and adequate strength for the purpose
for which it is used.
Inspect ladder before & after each use.
Do not use missing, broken or defective rungs, broken / split side-rails and faulty / defective
ladder.
Set ladders on firm surface. Check work area condition before work commencement.
Do not carry anything that will prevent work holding on with both hands
Do not tip your toes or use the 2 upper rungs of the ladder.
Use buddy system to support ladder
Wear safety system to support ladder.
Wear safety harness to anchor to adequate anchorage where possible.
