SKILL: ADMINISTERING NEBULIZATION


OVERVIEW
Nebulizers disperse fine particles of liquid medication into the deeper passages of the respiratory
track, where absorption occurs. The treatment continues until all the medication in the nebulizer cup
has been inhaled.
PURPOSES:
1. To liquefy and remove retained thick secretions.
2. To relieve respiratory insufficiency due to Broncho spasm.
3. To correct the underlying respiratory disorders responsible for Broncho spasm.
4. When a person has an acute asthma attack.
5. When a person is in respiratory distress.

EQUIPMENT:
- Stethoscope
- Medication and Medication Card
- Alcohol wipe or soap and water (if necessary)
- Nebulizer tubing or oxygen hookup
- Air compressor or oxygen hookup
- Sterile Saline
- Mask
- Gloves
- Client’s record

5. Identify yourself, verify the client’s identity by using two identifiers.

6. Explain to the client what you are going to do, why it is necessary ,and
how the client can cooperate.

7. Provide privacy.

8. Hand rub and apply gloves( if required by institutional policy.)

1. Remove the nebulizer cup from the device and open it.
a. Place premeasured unit-dose medication in the bottom section
of the cup and add prescribed diluent, if required.
RATIONALE:
To get enough volume to make a fine mist, normal saline may need to
be added to the concentrated medication.

2. Screw the top portion of the nebulizer cup back in place and attach the
cup to the nebulizer.
a. Attach one end of tubing to the stem on the bottom of the
nebulizer cuff and the other end to the air compressor or oxygen
source.

3. Turn on the air compressor or oxygen.


a. Check that a fine medication mist is produced by the opening the
valve.

b. Have patient place mouthpiece into mouth and grasp securely


with teeth and lips.

4. Hold the nebulization in an upright position.

5. Instruct patient to inhale slowly and deeply through the mouth. (5 pts.)
a. A nose clip may be necessary if the patient is breathing through
the nose.

b. Hold each breath for a slight pause, before exhaling.


RATIONALE:
The medication comes in contact with the respiratory tissue and is
absorbed. The longer the breath is held, the more medication can be
absorbed.

6. Continue inhalation until all medication in the nebulizer cup has been
aerosolized (usually about 15 minutes)
a. Once the fine mist decreases in amount, gently flick the sides of
the nebulizer cup.

7. Observe for adverse effects during and after administration.


a. Check the heart rate after the treatment (for patients using
bronchodilator drugs)

8. Encourage the patient to cough after several deep breaths.


a. Reassess lung sounds, oxygenation saturation and respirations

9. Have the patient gargle and rinse with tap water after using the
nebulizer as necessary. (5 pts.)
RATIONALE:
Rinsing is necessary when using inhaled steroids, because oral fungal
infections can occur.

10. Remove gloves and perform hand hygiene.

11. Document properly including the time, date, route of administration,


medication given and patient’s condition before and after administration.
