    I - IDENTIFYING INFORMATION
    A. Title Math and Science Month Celebration 2023
    B. Date October 13, 2023
    C. Venue Rawis National High School Rawis, Laoang, Northern Samar
    D. Participant’s The action was spearheaded by the School Head, Science and Math
    Description Department Chair, Science and Math teachers, participated by the
    students.
    E. Activity The activity involved a series of engaging activities:
    Description Morning Session:
    Quiz Bee, Environmental Quiz, Techno Quiz, Collage and Scrap Making,
    and Poster Making,
    Afternoon Session: Zym Go and followed with the short Program and the
    highlighted with the Search for Ms. Science 2023 and Mr. Math 2023.
    Then followed by the awarding of certificates for the winners.
    F. Objectives To showcase the talents and skills of every student in terms of innovation
    and discovery. This also enables students to enjoy Science and Math
    concepts and to engage them on scientific issues.
    G. End of Activity ACR, pictorials
    Outputs
    H. Highlights: The Math and Science Month Celebration involved a series of engaging
    activities’ and kicked off with the short Program and the highlighted with
    the Search for Ms. Science 2023 and Mr. Math 2023. Then followed by
    the awarding of certificates for the winners. The activity lasted for a day.
    II – RESULTS
    Achievement of
    Objectives (in %) The activity is 100% successful. All the objectives were achieved.
    A. Participant’s The following were provided as evaluative observation of the
    Feedback participants:
    1.Design of the Event
    Students enjoy the different contest, some of them keep on joining
    the contest even though they were not a winner, and the important
    was they enjoyed themselves.
    Students-participants especially the awardee enjoyed the event, they
    received their awards with smile on their faces.
    2. Methodology
    Information dissemination
    The School Principal forwarded a memo to the Science and Math
    Dept. Chair about the Science and Math Celebration. The Science and
    Math Department chair facilitated the planning for the said activity,
    then disseminated to the advisers and the assigned coach and
    facilitator, facilitated the said activity.
    3. Management of the Activity
    The Math and Science Month Celebration 2023 was managed by the
    Science and Math Teachers Under the supervision of School Principal
    sir Danilo D. Cerbito.
    4. Resource Persons
    The Math Department Chair Sir Rex Arnold B. Irinco he delivered his
    speech very inspiring to the students.
    5. Audience Participation
    Give appreciation to the awardee with their parents through
    clapping hands
    6. Logistical Arrangement
    The event made use of the following supplies, materials:
     Tarp
     Sound System
     Bond papers and pens
