    AN ORDER ESTABLISHING THE BARANGAY REHABILITATION AND REFFERAL
    SANGGUNIANG BARANGAY DESK AND DESIGNATING THE BARANGAY DUTY OFFICERS OF BARANGAY
    MEMBER BAGONG SILANG, MABUHAY, ZAMBOANGA SIBUGAY

    MARIO B. ONTAL WHEREAS, Section 3 (A) of dangerous Drugs Boards (DDB) Regulation No.2 series of
    Sangguniang Member 2016, mandates that the whole process of voluntary surrender to authorities shall be
    documented properly by the Duty Officer (DO) of the Barangay where the individual
    LEONARDO S. DINGLASA surrendered;

    WHEREAS, Section 6 (A) of DDB Regulation No. 3 series 2017, provides for the
    establishment of a rehabilitation Referral Desk in every Barangay to endorse Drug

    Dependents to the duly authorized representative of DDB pursuant to Article VIII of r

    Republic Act 9165 and other pertinent issuances of the Board;

    JACINTA T. PERIGO WHEREAS, DILG Memorandum Circular No. 2017 -151 dated November 10, 2017,
    Sangguniang Mamber reiterates the establishment of A Rehabilitation Referral Desk, designation of Duty
    Officer, organization of House Clusters, and designation of cluster leaders;

    Sangguniang Member NOW, THEREFORE, I, RENATO P. TUMACAS, Punong Barangay, Barangay bagong
    Silang, Mabuhay, Zamboanga Sibugay by the virtue of the power vested in me by kaw,
    MANUEL DIVA do hereby order the following:

    SECTION 1. ESTABLISHMENT OF BARANGAY REHABILITATION REFRRAL DESK

    Sangguniang Member There shall be a Rehabilitation Referral Desk in the Barangay which shall be located
    inside the Barangay with the Duty Officer as the Officer in-Charge.

    The duty of the day Sangguniang Barangay Member and all Barangay Health
    IMELDA T. ALTOVEROS Workers are hereby designated Barangay Duty Officers.

    SECTION 3. ROLES AND RESPONSIBILITIES

    Barangay Treasurer 1. Man the Rehabilitation Referral Desk;
    2. Conduct an interview to solicit personal information from the surrendered
    3. Immediately inform the Punong Barangay in case the surrenderer is include in
    EDITHA T. REDUCTO the target list, wanted list, and watch list personalities of the law enforcement
    IPMR agencies;
    4. Refer endorsed drug dependents to the Municipal Anti-Drug Abuse Council
    (MADAC) for screening Test (ASSIST);
    5. Log and maintain a record a record book containing all personal information of
    surrenderers ;
    6. Ensure confidentially of all information of surenderers;
    7. Perform other duties and functions as may be deemed necessary.

    SECTION 4. EFFECTIVITY. This Executive Order shall take effect immediately.
