Sunrise Rescue consists of a group of volunteers that are committed to find the Foster NOTES:
perfect home for animals we have in our care. Owning a pet is a lifetime
commitment that requires responsible owners who will ensure pet has its best
life. We do our best to ensure that each person who adopts from us is aware of
the responsibility they are agreeing to take on. The adoption process takes time
and requires adopting parents be open and willing to give any information
necessary including having a home visit to make final decisions when applying to
adopt with us. Your application will be review closely, please answer all questions
and be prepared to provide supporting materials. We thank you for adopting
with us and look forward to pair you with your forever fur baby. WE require all
adopters be 18 years or older to adopt an animal.

You are assuming responsibility for all care related to Maltese 1 Pup.
You agree to make every and all necessary Veterinary appointment on a timely basis.
You agree to assume responsibility to get any and all required treatments, vaccine as recommended
by his licensed veterinary.
You agree that you will ensure that Maltese 1 Pup is well maintained and in good health condition.
You agree to pay any and all Veterinary fees accrued by foster parents.
You agree to pay any adoption fees if any are required. Fees as of 2/17/2023 are $275. plus neuter
fees. These fees are subject to change until adoption is finalized. Neutering fees shall be paid to
veterinary directly
You agree neutering will be done no later than six months after adoption date.
You agree to pay a $1,700. penalty fee to foster parent if Maltese 1 Pup has any liter before
neutering. The $1,700 will be charged for each liter the unaltered Maltese 1 Pup is responsible for.
You agree and release the foster parent from any and all liability and responsibility for any and
all medical treatments or ailments minor or major Maltese 1 Pup should be diagnosed with or
have.
You agree to contact foster parent immediately and before making any final decision of
rehoming or getting rid of Maltese 1 Pup if for any reason you decide to rid of or cannot care for
him.
You agree that any rehoming will be done by the foster parent only and agree to pay a penalty
fee if you can no longer care for Maltese 1 Pup.
You agree to contact the foster immediately if Maltese 1 Pup goes missing and agree to pay a
penalty fee if you fail to do so.
You agree to ensure that Maltese 1 Pup is well protected and does not get in a situation where
he is able breed.
You agree to take Maltese 1 Pup to the Veterinary immediately after adoption for updates on
any vaccines or treatments needed and agree to assume all billing responsibility.
I agree to register Maltese 1 Pup's microchip with the microchip company and notify the foster
parent and that company within a week of my address/phone number changes.
You agree to make your home available for a home visit and that all family members who live in
the household will be available during visit.
I certify that all the above information is true and accurate. I understand that giving any false
information or refusing to provide supporting materials can result in denial of application. I understand
Sunrise Rescue/foster parent/its volunteers reserves the right to deny the adoption of any animal. I
acknowledge that information regarding the history, health, and behavior of adopted animals may not
always be available or accurate.
