    (formerly known as CGS-CIMB Securities Sdn. Bhd. (“CGS MY”) and hereby confirm:

    a) In addition to the trading account stated above, I would like to apply to open a Securitised
    Trading Account - Nominee with CGS MY (“ESOS Account”) solely for the purpose to
    custodize the shares pursuant to the exercising my employees’ share option scheme (ESOS)

    b) I authorize CGS MY to rely on the information utilized to open my Direct Trading Account, for
    the purposes of the application of my ESOS Account and I agree that all terms and conditions
    in the Application Form for the purposes of the opening of the ESOS Account,
    c) I instruct CGS MY to close my ESOS Account upon the expiration of ESOS entitlement AND
    there is no remaining securities or monies in the ESOS Account, whichever later.

    I confirm have read, understood and agree to be bound by CGS MY’s General Terms and
    Conditions and Risk Disclosure Statement (which is available on https://itrade.cgsi.com.my/ and
    may be updated from time to time) for the maintenance of the ESOS Account with you.

    I will not claim against you for all or any matters arising out of this Letter of Authorization.

    I declare that you should not be held liable or responsible to me for any loss or damage howsoever
    arising (to the extent permitted to exclude such liability by law) as a result of any act, neglect,
    omission or default wilful in connection with this Letter of Authorization.

    I shall at all times fully and effectively indemnify you, your affiliates and your respective directors,
    employees, agents and representatives (“Indemnified Persons”), and keep the Indemnified
    Persons fully and effectively indemnified and hold harmless from and against all actions, suits,
    proceedings, claims, demands, losses, charges, penalties, fees, fines, costs and expenses
    (including but not limited to legal fees and costs on a fully indemnity basis) whatsoever which you
    may incur as a result of or arising from your acting under or in connection with this Letter of
    Authorization.

    I will ratify and confirm all and whatsoever the cause to be done by virtue of this Letter of
    Authorization and I hereby declare that any act or thing done after the revocation of this Letter of
    Authorization by my death/insolvency or otherwise howsoever shall be valid unless notice in
    writing of such revocation has been received by you.

    In the event of that any one or more of the provisions contained in this Letter of Authorization shall
    for any reason to be unenforceable illegal or otherwise invalid in any respect under the law of
    Malaysia, this Letter of Authorization shall then be construed as if such unenforceable, illegal or
    invalid provisions had never been contained herein.
