I have over 10 years experience in Palliative Medicine in England. I completed Core Medical Training and MRCP UK in 2011, and Specialist Training
in Palliative Medicine to ST6 level with the SCE Palliative Medicine in 2020. I submitted CCT in Palliative Medicine via the Portfolio pathway in
November 2023. I deliver complex symptom management for the frail elderly and patients with organ failure, neurodegenerative diseases and
advanced cancer diagnoses.
I am trained in trauma-informed talking therapies which help with complex advanced planning conversations, carer support and MDT working.
I use validated patient outcome measures (IPOS) and other OACC Suite tools from my MSc module training at King’s College, London. I was
awarded the “IPOS Champion” at The Royal Marsden NHS Trust as a training registrar.
In my current post, I set up an IT web-based platform for sharing digital project information and maintaining written clinical handover of patients’
clinical care in real time. This has improved efficiency in team working and after-hours handover while maintaining confidentiality. I led an audit on the
adherence of diabetes management towards the end of life. The audit loop shows adherence to safe clinical practice against 2022 national guidance.
The outcome of the audit will initiate the use of technological advances of non-invasive blood sugar measurements.
I completed a systematic review to update local guidance on the use of denosumab for skeletal-related events as part of MSc Module at King’s
College, London in evidence-based guidance development. I received a distinction for this piece of work. I led a trust-wide audit on the hidden
mortality and cost of low falls, and this was presented at a national specialist meeting. Change management from the audit and the reduction in
patient morbidity and bed occupancy led to trust savings. I met educational targets for the same service by setting up interventional skills laboratory
teaching for medical students and junior doctors.
I advocate for doctors with differential attainment due to disability. I promote inclusive practices to improve healthcare outcomes for patients from
diverse backgrounds with complex health needs. As a result of my teaching initiative on Autism aimed at Doctors at Royal Trinity Hospice, National
Autism Training is now mandatory training in line with national objectives for the NHS. I lead reflective practice Balint Groups to promote healthy
teamwork and will present data from the pilot at the next Hospice UK national meeting.

The Whittington Trust was where I had my first employment as a clinical fellow in my first post in the NHS. I enjoyed working within the diverse
population the Whittington served and the wonderful teaching experience I received from clinical staff I worked with. The Whittington has a reputation

for inclusive clinical leadership and I hope to have the ability to gain leadership experience and receive mentorship as I complete my New Consultant
Training programme with the FMLM.
I hope to maintain my clinical capabilities towards completion of CESR (now portfolio) CCT route submitted in November 2023 Palliative Medicine. I
am able to deliver symptom management for the frail elderly and patients with organ failure, neurodegenerative diseases and advanced cancer
diagnoses. I demonstrate leadership through effective interpersonal relationships in multidisciplinary team work and coordinate holistic patient care
and discharge planning with the community services in the local area. I know to demonstrate effectiveness of service through using validated
outcome measures and adherence to good clinical practice through audit, and to participate in any national benchmarking measures. While I am very
much of the idea that I will concentrate on providing excellent clinical care, I have lessons from working at an Outstanding CQC rated service and
under mentorship of inclusive leadership which I can bring to the current clinical team.
II/ Details of any Relevant Experience Gained in Paid or Voluntary Capacity
I have over 10 years experience in Palliative Medicine in England supported by specialist training in palliative medicine with a training number in the
NHS. I completed Core Medical Training and MRCP UK in 2011, and Specialist Training in Palliative Medicine to I have over 10 years experience in
Palliative Medicine in England. I completed Core Medical Training and MRCP UK in 2011, and Specialist Training in Palliative Medicine to ST6 level
with the SCE Palliative Medicine in 2020. I submitted CCT in Palliative Medicine via the Portfolio pathway in November 2023. Audit projects I have
lead which resulted in change of team management include Management diabetes at the end of life in line with Diabetes UK national guidance and
electronic prescribing discharge summaries in line with Royal College of Physicians good practice guidance for safe discharge.
III) Details of any training, education or other interests relevant to the post
I am enrolled in the FMLM New Consultant training programme to help with leadership goals. I have completed conflict resolution training with the
MDU and On the Job Teaching Educational Supervision course with the Royal College of Physicians. I lead Balint Groups, and present positive
educational feedback for conducting them within the Hospice organisation at Hospice UK. Other further training include accreditation in advance
specialist psychological talking therapy Dialectical Behaviour Therapy.
IV) Personal Attributes
My specialist interest is the overlap of mental health and palliative medicine. As a portfolio doctor I account for and advocate for this population of
patients. Inclusivity includes being accountable for my own health and supporting and managing my disability effectively through education and
collaborative team working. Through my initiative, I implemented mandatory training on Autism for all employees for in line with the NHS standards. I
participate several compassionate initiatives in the local community including outreach talks on death and dying and running death cafes.
