    MONITORING TOOL ON THE PRODUCTION AND STORAGE OF SLMs/LAS


    Directions: Please check the appropriate description that best reflects your practice/s on each activity enumerated below regarding
    the learning resource processes expected to be done in the school level.
    The meaning of the descriptions are as follows:
     VERY EVIDENT- the activity is regularly done and there is an obvious manifestation that the activity is
    habitually done. Thus, the activity becomes a standard.

     SLIGHTLY EVIDENT- the activity is sometimes done and has manifestation that the activity is conducted in
    some instances. Thus, the activity is not an observed standard.

     NOT EVIDENT-the activity is not observed because it is not done.

    1. Reproduced SLMs aside from the SLMs provided by SDO Inventory of SLMs
    reproduced by subject and
    by grade level (Kinder to
    Grade 6; Grade 7-10, SHS)
    2. The school provides funds for the reproduction of the teacher- ADAS report on the
    developed LRs. summary of procurement of
    supplies for the reproduction
    of SLMs per quarter
    3. The school has equipment in printing/reproduction of the teacher- List of available equipment
    developed LRs (e.g., computers and printers) for printing and reproduction
    4. The school compiles, collects and keeps all the developed Soft copy of available
    LRs/SLMs/LAS for future use. SLMs/LAS in all grade level
    (stored in the
    USB/CD/external
    drive)/Inventory of
    SLMs/LAS received per
    teacher or prepared by the
    LR Coordinator
