    1. I, hereby, declare that, the entries made by me in the Application Form are complete and true
    to the best of my knowledge and based on records.

    2. I, hereby, declare that, I will be punctual and regular in attending classes, tutorials,class tests
    examinations, practical’s etc. Failure to obtain the required attendance percentage, non-
    submission of assignments and unjustified absence from tests /examinations will be treated as
    breaching the code of conduct.

    3. I, hereby, declare that, I will be seated in the classrooms at the beginning of each period. I
    will not enter or leave the class room without permission of the faculty.

    4. I, hereby, declare that, I will not make use of Mobile Phone during class hours as it is
    prohibited and is liable for punishment.

    5. I, hereby, declare that, I will carefully follow the Notices put up on the Notice Board (both
    online and offline). No excuse will be given by me if I ignore Notices on the Board.

    6. I, hereby, declare that, in absence of teacher in the classroom, I will maintain a strict order
    within myself so as to not disturb the adjoining classes. Under no circumstances I will leave the
    classroom without ascertaining the relieve orders from the Staff room.

    7. I, hereby, declare that, I will attend College with suitably descend dressed.

    8. I, hereby, declare that, I will wear college Identity Card or carry library card in the College
    hours inside the college campus.

    9. I, hereby, declare that, I will not loiter or stand near the verandas or corridors of the College
    during lecture hours.

    10. I, hereby, declare that, I will not invite friends from other institutions and outsiders to the
    College

    11. I, hereby, declare that, I will not paste or fix with cello tape anywhere on the College walls
    any writing, scribbling on the walls which will attract disciplinary measures against me.

    12. I, hereby, declare that, I will not do Smoking, Chewing and Spitting Tobacco products or
    pan masala, consumption of alcohol or narcotic drugs as it is strictly prohibited inside the
    college premises. Violation of this rule will invite me with disciplinary action as the college
    premises is a smoking free zone, and I will respect it.

    13. I, hereby, declare that, I will not commit or cause breach of peace inside and around the
    College campus.

    14. I, hereby, declare that, I will not be indulging in any games, even in small groups, anywhere
    in the College premises during lecture hours.

    15. I hereby declare that I will neither join in any agitation/strike for the purpose of forcing the
    authorities of the college to resolve any problem, nor I will participate in any activity which has
    a tendency to disturb the peace and tranquillity of academic environment of the college campus
    and/or its Hostel premises.

    16. I hereby declare that I will treat the members of the staff teaching and non- teaching and my
    fellow students with courtesy both in and outside the College. I will not, by my act or conduct,
    cause damage to the reputation of the college.

    17. I hereby declare that I will not conduct any malpractice (copying, impersonation, use of
    unfair means, exchanging answer-sheets, and use of digital device, etc.) during examination
    which will invite me with severe disciplinary action

    18. I, hereby declare that, both I will not indulge in, nor tolerate ragging, in any form, even in
    words or intentions, and I accept to give an undertaking in the prescribed format for the same.

    19. I, hereby declare that, I shall be solely responsible for my involvement in any kind of
    undesirable / in-disciplinary activities outside the campus, and shall be liable for punishment as
    per the law of the land. I, further understand that, the college shall in no way provide any
    support to me and will not be held responsible for my any such action.

    20. I, further declare that, my admission may be cancelled, at any stage, if I am found ineligible
    and/or the information’s provided by me are found to be incorrect.

    SCOS also reserves the right to cancel admission of any student and discontinue his/her
    studies at any stage of studentship for the unsatisfactory academic performance and /or
    any kind of undisciplined conduct.
