An OSH/EHS Joint Walk carried out at Yas Living on 04th July 2023 at 10:00 am in line with ALDAR
Proactive Monitoring Programme in combining with Arcadis and Fibrex representatives were present
during the walk and the following observations have been highlighted by Arcadis and appropriate
actions shall be taken by Main Contractor according to the response category and across the whole
project.

Observation Category: Excavation (NDRC)
Risk Category: High
Observation Description: Extremely unsafe work practices at the
external area and NOYA 3 NDRC pits related to:
A. Inadequate ventilation inside the NDRC pit in NOYA 3 wherein
1 the ventilation ducting did not extend till the bottom of the pit
thereby exposing the workforce to unsafe work conditions Fibrex A
which can result to serious incidents.
B. Used of unsafe straight ladder for access noted inside the pits
at the external project area. Used for straight ladders is
prohibited to be used as access.
C. Used of invalid PTW for lifting without an assessment of the

risk involve with lifting materials/equipment such as material
basket, etc. Carrying out lifting activity without properly
assessing the risk might result to serious incident.
D. Poor cable management where energized electrical cables are
lying on the ground and on scaffold pipes.
Actions to be taken as agreed (but not limited to):
1. M/s Fibrex is therefore advised to take practical preventive
and corrective measures to control the risk related to
excavations at the NDRC pts at the external project and
NOYA 3 by taking proactive safety control measures while
considering:
i. Ensure that the ventilation duct extend right till the
bottom of the pits.
ii. Use of suitable access and egress. Straight ladders
shall be prohibited.
iii. Ensure proper cable management by rooting the
cables beneath the ground of suspend them to avoid
electrical shock.
2. PWT system shall also be implemented effectively and ensure
that risk assessment must be carried out for lifting and other
major high-risk activities. RA should be attached to the PTW.
3. OSH information, instruction and training shall be adequately
communicated to all the involve parties along with
deployment of full-time competent supervisor to oversee all
activities related to NDRC.

General OSH / EHS issues:
A. Use of Unsafe Safety harness: NOYA 1: Lack of inspection and
maintenance of safety harness noticed at the external project
area whereby defective harness have been used with damage
hooks (Attached Photos No. 5&6)
B. Barricading Hazards (Trenches): Location: NOYA 3:
2 Fibrex B
Unguarded trench edges noticed along several trenches in
NOYA 3. Moreover, that is lack of directional/cautionary signs.

C. Use of Flagman: Location: NOYA 1 & 3: Several mobile
equipment noticed without designated flagman moving along
pedestrians’ roads which can result to serious incident.

D. PPE: LOCATION: NOYA 2: Several workers noticed at site
without using proper PPE. (See Attached Photos No. 10)
Actions to be taken as agreed (but not limited to):
1. The contractor to engage its subcontractors to carryout
periodic inspection and maintenance of safety harnesses and
other gears used at site.
2. Ensure that all excavations/trenches are fully protected with
edge protections along with providing visible signages.
3. Each equipment shall be provided with a certified and trained
flagman to direct the equipment in areas where pedestrians
are moving.
4. All workers are required to use mandatory PPE while at site.
PPE should be control at the security guard and no worker
should enter the project without PPE.
