Capt. Lepon: Good day everyone! I call this meeting to order. My name is Capt.

Clint John
Lepon, and I will be presiding over today's proceedings. I'd like to extend a warm welcome to all
participants present here today. Thanks for being here. We're right on schedule to start this
meeting, so let's go ahead and dive in."

Capt. Lepon: Before we start 1st officer Ando will be conduct a attendance, raise your hands if

Capt. Lepon: The purpose of this meeting is to discuss about last incident in vessel of Kota
Bahagia. We have a well-prepared agenda that will guide us through the topics we need to
cover. I will now share the agenda for today's meeting.

Capt. Lepon: Our first item on the agenda today is "incident overview". This item is important
because here we will know what happened and how it started.
Capt. Lepon: To initiate our discussion, I would like to invite 1st officer Ando to provide a
presentation or share any necessary information regarding this agenda item.

1st officer Ando: So the first agenda is the incident overview the question for that what is
happened?

1st officer Ando: At 1700 on 17 December 2020, the Kota Bahagia berthed alongside at Wharf
4, Napier Port, New Zealand. The cargo for discharge at Napier included wind turbine
components that had been carried from Taicang, China.

1st officer Ando: Shore-based staff, including stevedores1 and fitters2 from a local engineering
company, boarded the vessel to commence unlashing the cargo in preparation for
discharge.Cargo-discharge operations commenced at 2212 and continued overnight.At about
0648 on 18 December 2020, two fitters from a local engineering company commenced hot
work in number 2 cargo hold ‘tween deck

1st officer Ando: After discharging one container, a stevedore noticed white smoke in cargo
hold number 2. The stevedores and fitters in the hold discussed whether it was dust or smoke.
The fitters had finished their work in the 'tween deck and were stowing the gas-cutting
equipment. Shortly after, the stevedore confirmed that it was smoke. Around 10:39 AM, as the
crane driver returned the container spreader to the cargo hold, the smoke turned black and
thickened, and the workers realized there was a fire. Within 10 to 20 seconds, the smoke
became very thick, forcing the workers to pass near the fire to escape the hold.

1st officer Ando: After the investigation they say the cause of the fire is Molten material,
ejected during gas-cutting activities, very likely ignited dry sawdust nearby, which created a
smouldering fire that ignited the polyvinyl-chloride tarpaulins. And other combustible
components of the fibre-glass project cargo.And that's all the incident overview.

remarks action take for this?

the company owner, prompt communication was initiated regarding the incident. Here are the
remarks on the actions taken:

company owner about the fire incident in cargo hold number 2. The communication included
essential details such as the occurrence of the fire, the actions being taken, and the potential
impact on cargo and vessel operations.

owner, ensuring transparency about the situation. This included sharing information about the
progress made in firefighting and containment efforts, as well as any challenges or
developments that arose during the incident response.

significant importance and requires our attention.

Capt. Lepon: During our discussion of the second agenda item, we would greatly appreciate

knowledge in this area, we would like to invite you to share your thoughts and lead the
discussion on this particular topic.

Safety officer Paragados: Our 2nd agenda is the Impact assessment to discuss what was the
impact of incident in our vessel or cargo.

Safety officer Paragados: Safety operations?, Environment?, Reputation?, thats our discussion
for this agenda.

Safety officer Paragados: for the safety operations there are so many impact but for the
common is Human safety: Assessing the potential impact on the safety and well-being of
personnel involved in the operations. This includes evaluating risks related to injuries,
accidents, occupational hazards, and health impacts.

Safety officer Paragados:for the next is environment. What is the impact for environment? the
potential consequences on the environment, such as pollution, contamination, or ecological
damage. This includes assessing risks related to spills, emissions, waste management, or other
activities that may have adverse effects on ecosystems.
Safety officer Paragados: there's a possibility that all scenarios can happen so we need to
prevent safety precautions.

Safety officer Paragados: And the last what impact for our Company reputation? Trust and
Confidence: Safety incidents can erode trust and confidence among our stakeholders, including
customers, clients, partners, and investors. They may question our commitment to their well-
being and doubt our ability to deliver on our promises.And that's all for 2nd agenda discussion.

all for this. 1st is safety operation Conduct additional training and drills for the crew to enhance
their emergency response capabilities for once a week.

technologies and practices, such as alternative fuels or ballast water treatment systems.

Capt. Lepon: And the last for Company reputation is Enhance corporate social responsibility
initiatives to demonstrate a commitment to community and environmental well-being.

3rd officer Vilbar: In my experience in incident is our Risk assessment: Conducting thorough risk
assessments to identify potential hazards and vulnerabilities is crucial. Understanding the
specific risks and their potential impacts can help prioritize resources and develop effective
response strategies. Because we have a lack for inspection or understanding.

3rd officer Vilbar: So I suggesting is to 1st Conduct regular drills and exercises to ensure the
crew is well-prepared and familiar with emergency response procedures . And 2nd Review and
update emergency response plans based on lessons learned from the incident and industry best
practices. And last Provide comprehensive training to the crew on emergency response
protocols, including evacuation procedures, fire-fighting techniques, and first aid.

Capt. Lepon: Next summarize in all first is the incident summarize On December 17, 2020, the
Kota Bahagia arrived at Wharf 4, Napier Port, New Zealand, at 1700. Among the cargo unloaded
at Napier were wind turbine components transported from Taicang, China. The investigation
determined that the fire was caused by molten material expelled during gas-cutting operations.
This molten material likely ignited nearby dry sawdust, leading to a smoldering fire that
eventually set fire to the polyvinyl-chloride tarpaulins.
