PROGRAM OF ACTIVITIES
“A Covid 19 Response to Community Wellness: A Feeding Program to Selected
indigent Children”

DATE: January 2020 – January 2022

Venue: Barangay Lourdes

January 2020 – February 2020

A conference with the purok Chairman, Barangay Officials and the SK Officials was
held regarding the “A Covid 19 Response to Community Wellness: A Feeding Program to
Selected indigent Children” of Lauc Pao Community within the barangay. The activities
include organization of the different committees, discussion about the projects. After the
discussion the survey of the site was done.

July 2020 – August 2020

Implementation of the said project begins. All the members actively participated.
They started to collect the target list of the indigent children in the Barangay.

September 2020-October 2020

A supervisory plan for the members was prepared, and a conference via
Google Meet was held to inform the purok chairman of the schedule for feeding supplies. After a
brief instruction, the supervisory plan was finalized. Purok chairman and members actively start
listing and collecting the indigent children. They weighed all the indigent children and submitted
the Nutritional Status of Indigent Children. They inform the families of the beneficiaries of the
feeding supplies to be given to them. The said virtual meeting was explained clearly and well by
the purok chairman.

November 2020 – April 16,2021

Purok chairman and members actively start the distribution of the feeding
supplies to the selected indigent children.

January 2021

Another virtual conference via fb messenger was held with the members and
purok chairman for the inter kamustahan for selected indigent of every Purok .
February 2021

The result of the inter kamustahan via fb messenger was well done. Every Purok
participated for the said projects. The chairman discuss again for the second evaluation of the
distribution of feeding supplies project. At the end of the month there should be a constant
monitoring of body weigh done at home to monitor the improvement of selected indigent
children.

March 2021

April 2021 – December 2021

All the activities were accomplished successfully. Purok Chairman and members
were carefully watched and instructed to have a constant monitoring for the selected indigent
children.

January 2022

For the final evaluation, the Chairman gathered the body mass index of selected indigent
children. From the result it was found out that 75% of the indigent gained weight. Also, almost
90% of them have a normal body mass index. The success of this project should be determined
by the improvement of the children from the first time they were weighted.

ACCOMPLISHMENT REPORT IN CONDUCTING THE “A COVID 19 RESPONSE TO

COMMUNITY WELLNESS: A FEEDING PROGRAM TO SELECTED INDIGENT CHILDREN”
PROJECT

SY 2020 - 2022

At the span of two years the following were our accomplishments in conducting a Feeding
Program Project in our barangay Lourdes.

1. Parents of the selected indigent children were well encouraged and motivated about the
importance of the nutritional status of their children.

2. At the start of the project there were 20 beneficiaries of the feeding program. The feeding
supply were distributed in the barangay and the parents of every beneficiary were instructed to
get the feeding supply allotted to their children.

3. Constant monitoring of the BMI of selected indigent children.

4. With the implementation of the projects, the parents of the selected indigent children were
motivated and received additional support to the said project.

Indeed, the success of this project was determined by the improvement of the children from the
first time they were weighted.
