SEQUENCE OF BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDED ACTION OR PROCEDURE

Erecting and turning shell plates Tipping of the crane Make sure that the crane is level and the outriggers are
fully extended before start lifting any thing.

Use approved crane pads before any lift.

Riggers are always required to assist the operator by


checking the ground, setup and surrounding areas
during positioning of the crane.

Mechanical failure Daily inspection of the crane every morning before start
using it.

Complete the ECBI daily checklist.

Un balanced load Use taglines to control the load.

Pinch points Keep your hands and body parts away from the pinch
points during erecting the plates and turning it.

Side loading the crane Position the crane in such away that it don’t side load
due to turning of the plate.
Rigging shell plates Rigging failure Daily inspect the rigging equipment and check if it is
recently color coded.

Do not use defective rigging tools. If any defective tools


noticed red tag it and take it out from service.

Over loading of the crane Check the load of the plate to be lifted and confirm the
rigging equipment to be selected according to the load
and the crane capable of lifting the gross load.

Falling objects Place the materials in the tools baskets before taking
the plate to the place.

Tools and fit up materials to be stored inside the tool


pails.

Clamp slip off Use the clamp as per the manufacturer requirements
and CBI standards.

Working at heights Workers are required to use the CBI approved safety
harness with shock absorbers and two lanyards. 100%
TIE off is required.

Scaffold and ladders to be used should have a green


tag and inspect by a competent person.

Jumping of scaffold from one ring to another Falling from height Workers are required to use approved anchorage
points such as 3 sides weld 3/4 “ nut or over the shell
hook device or miller retractable life line.

Yoyo should be installed on the vertical shell ladders.

When starting the transfer of the scaffold from one to


another one end should be hard barricaded for
restriction of access.

Falling objects Materials and tools should be stored in the tool pails.

Overloading of scaffold Materials should be placed near the bracket arm area
Avoid stacking of material at one point.

Not more than 3 people to stand on one span of the


scaffold.
Shifting of materials Sharp edges Use working gloves and watch for the sharp edges.

Improper rigging Provide proper rigging as per the instructions of the


foreman.

Rigging failure Check the capacity of rigging as per the load. Inspect
all rigging tools prior to use. Remove any defective
rigging tools from service.

Use softeners at sharp edges at the connection point
with the slings.

Back strain Use proper lifting techniques.

Load falling from the trailer Properly secure and tie the load on the trailer.

Load falling from the crane Do not lift un even loads higher than required. Keep the
load lower as possible.

Do not walk under the suspended load.

Swinging of the load Use tag line to control the load from swinging.

Pinch points Protect your hands and other parts of the body from
the pinch points. Use hand gloves.

Stay away from the crane while the crane operator


extent or retract the out riggers.

Crane failure Daily inspection of the crane every morning before start
using it.
