    PS563 Unit 8 Assignment (Behavior Intervention Plan) Template

    Introduction

    You should provide a theoretical introduction here that discusses the importance
    of a behavior intervention plan and the components of the plan. Be sure to cite
    accordingly and double space as per APA 7 th Edition formatting requirements.

    Target Behaviors

    (1 behavior per bullet with definition & rationale for why it was chosen to target)
     BEHAVIOR: Definition. Rationale for chosen for intervention.

    Summary of FBA Results

    Provide a concise summary paragraph discussing the results of the FBA and the
    hypothesized function of the target behavior(s).

    Treatment Goals

    (1 goal per bullet; make sure these are measurable, i.e., include numbers!)
     Long-term goals (at least 2)
     Short-term goals (at least 2)

    Intervention Strategies

    (1 intervention strategy per bullet; written technologically and clearly; provide


    enough detail so that someone could provide the intervention to integrity with just
    this information; provide at least one peer-reviewed source per intervention to
    support your use of it)
     Antecedent interventions
     Consequence interventions

    Data Collection & Measurement

    Write a paragraph about how you will collect data on your target behavior to
    measure intervention progress (e.g., what kind, how often, etc.). Also, specifically
    state how you will use the data collected to assess your client’s progress
    throughout intervention.
