    It is the responsibility of the employee to indicate what period they wish to be assessed on. Please
    indicate which period you wish to be assessed on:

    If you are being assessed on last year the period should be 01/01/2020 to 31/12/2020

    If you are being assessed from the beginning of the year the period should be 01/01/2021 to last Sunday.

    If you are being assessed on the last four weeks the period should be the four weeks up to last Sunday,
    i.e. Monday of week 1 to Sunday of week 4.
