Gross: Specimen container is labeled with the patient's name and medical record number.
Received in formalin is distorted two fragments of uterus with attached right ovarian cyst and fallopian
tube is previously incised. Larger fragment measures 14 x 9.5 x 7 cm. Smaller fragment measures 9.5 x
8.5 x 5.5 cm. Ectocervix is shaved off. Cervix measures 1.8 x 1.6 cm. The endocervical canal measuring
1.5 x 1.0 cm. Endometrial cavity measuring 4.0 x 2.0 cm. Maximum thickness of endometrium is 0.2 cm
and that of myometrium is 6.0 cm. Endomyometrium shows adenomyosis and single subserosal and 0.7
to 3 cm. Right fallopian tube measures 2 x 0.5 cm. Previously incised right ovarian cyst measures 5.5 x
4.5 x 3.0 cm. Fimbriae not identified. Cut surface of fallopian tube is grossly unremarkable. Cut surface of
ovarian cyst shows chocolate brown material. Maximum thickness of cyst wall is 0.7 cm. Representative
sections are taken and submitted as follows:-
A) Anterior and posterior cervix with fibroid; B) Anterior and posterior endomyometrium; C) Submucosal
fibroid; D) Section for adenomyosis; E) Ovarian cyst and fallopian tube; F-H) Probable fallopian tube.
Micro: Sections show a cyst lined by columnar cells showing basally placed nuclei surrounded by endometrium
type stroma. Hemosiderin laden macrophages are also present.
Diagnosis: UTERUS WITH RIGHT ADNEXA, TOTAL ABDOMINAL HYSTERECTOMY WITH RIGHT
SALPINGO-OOPHORECTOMY:
Endocervix: Leiomyoma, 0.7 cm.
No dysplasia or malignancy.
Endometrium: Cystic atrophy.
Myometrium: Extensive adenomyosis.
Right ovary: Endometriotic cyst.
Note: Right fallopian tube is not identified.
