LEARNING FEEDBACK DIARY
C- The community duty was conducted for 1 day in Indahag in order conduct a home visit as well as the
preparation prior to this which is the presentation of the FCP’s, TLG’s and nursing interventions in the
previous day. The given FCP format was filled in to create a systematic guide to identify health and
nursing problems, and plan appropriate goals and objectives, nursing interventions, and other
components needed to minimize or eliminate the identified problems. The TLG served as a complement
to the health teachings. There were also pre- and post- conferences in which our clinical instructor
discussed the objectives for the day, reminders, and asked for our insights during the lecture as well as
sharing of the group’s clarifications regarding the community duty and requirements.

E- During the community duty, new insights and relationships were formed. At the start of the
community meeting, I was able to establish rapport with my assigned family as well as became closer
with my groupmates. I practiced the procedures for home visit, bag technique, and health teaching
based on the priority family nursing problem identified. There was also a sense of comfort and openness
throughout the family and group meetings as each of our presence, thoughts, and feelings were
regarded. Along the conversations, I was able to learn about broad and key information regarding the
background, usual events and circumstances within the family which were essential for the making of
the FCP. During the finalization of the FCP’s and TLG’s our interventions and FCP’s were interrogated;
me and my group collaborated to decide on the appropriate answers that would represent the group’s
efforts. We also took note of the corrections and gathered our questions so that all of them would be
catered in the Q&A portion especially regarding the duty requirements.

R- At the end of the community duty, through the insights and relationships formed, I felt more the
reality and responsibilities as a student nurse regarding the discipline of community health nursing. The
activities of the community duty, specifically the performing of the nursing interventions, made me
reflect on my knowledge and skills to perform basic nursing procedures to my clients and create a FCP
and TLG in which the quality of care and health education given would be applied. The activities
reminded me of what is expected just like complying the requirements with quality and studying well.
However, I am reassured as I am not alone because I am within a group in which we have good rapport
as well as be guided by my clinical instructor along the way.

A- After reflecting, I am able to come up with thoughts that would act as motivation and solutions as I
encounter the upcoming challenges in my succeeding community duty experience. I should note that no
matter how many corrections, moments of anxiety or stress, these are part of the training process to
become a good community health nurse in the future. Also for the same reason, I have to continue
doing my best studying and developing my critical thinking skills in complying the case studies, reports,
and other requirements, as well as practice teamwork with my RLE group members.

E- Evaluating the actions decided, I think these are possible measures I can do to mature as a
sophomore student nurse as based from experience; perseverance has never failed to make people
achieve their goals or at least have personal growth. Although it is difficult, it will be more bearable with
a supportive environment which includes my parents, friends, classmates, clinical instructors, and God.
