    Dear Respondents;

    This is to introduce our group from 3rd year Bachelor in Early Childhood Education
    students of Institute of Creative Computer Technology Colleges Foundation, INC. (ICCT
    Colleges) Cainta Main Campus who intends to conduct survey in regards to our study
    entitled “Assessing the Impact of COVID-19 Pandemic on Student Well-being at De
    Castro Elementary School”. Our group is in need to ask for your knowledge and
    experiences that would be a big help in the success of our study. 

    We are requesting for your pure honesty in answering the questions based on your
    personal view and observations. You have the right to not indicate your name and
    personal profile for confidentiality of your identity. 

    This is in accordance with Republic Act No. 10173 of the Philippines, Data Privacy Act
    of 2012 all personal and/or sensitive information solicited and disclosed from these
    questionnaires shall be only for the study alone and will remain confidential. You have
    the right to sue us if ever we leaked information that will offend your personal life.
