    QUARANTINE CERTIFICATE

    This is to certify that the bearer SANTIAGO, JOLEE ESPIRITU with residence at TUCOP, DINALUPIHAN,

    BATAAN has completed the Mandatory Quarantine Process and COVID-19 FREE as reflected in the result of the

    Reverse Transcription – Polymerase Chain Reaction (RT-PCR) after testing and laboratory report of the JOSE B.

    LINGAD MEMORIAL GENERAL HOSPITAL on SEPT. 11, 2020.

    This certificate is issued after a thorough medical examination determining that the bearer has no

    medical signs and symptoms related to COVID-19 and is hereby cleared to leave the quarantine facility and

    return to residence as stated.

    Given this day SEPT. 12, 2020 in the City of Manila, Philippines.
