    JOINT - AFFIDAVIT

    We, ANTONIO QUISAY, JR., JESS BALBONTIN and KIMBERT BILBAO, all of
    legal age, Filipinos, all single and residents of Barotac Nuevo, Iloilo, Philippines, after
    having been duly sworn to in accordance with law do hereby depose and say:

    1. We are the Complainants against Dr. Ramon Zarceno in (a) OMB-V-C-16-


    0349 to 0351 for Violation of R.A. No. 7877 and Violation of R.A. 7610, and (b)
    OMB-V-A-16-0424 for Grave Abuse of Authority and Grave Misconduct before
    the Office of the Ombudsman;
    2.

    2. That the afore-mentioned license and card was kept inside my wallet, but the
    said wallet was lost while I was riding a Jaro-CPU route jeepney from Ungka Terminal
    going to Iloilo City last October 20, 2019 and I only noticed that it was gone when I was
    already home;

    3. That I exerted diligent efforts to look for the said driver’s license and ATM card
    but to no avail hence, I consider the same already lost and beyond recovery;

    4. That I am executing this affidavit to attest to the truth of the foregoing and to
    declare that the said Driver’s Lincense and ATM Card is already lost and beyond
    recovery and to request the Land Transportation Office - Iloilo and the Land Bank of the
    Philippines - Solis St., Iloilo City Branch , to issue a new Driver’s License and ATM
    Card, respectively, in my favor; further, I am executing this affidavit for whatever legal
    purpose it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 24th day of October
    at the City of Iloilo, Philippines.
