    AFFIDAVIT OF LOSS

    I, VICTOR L. CARO, of legal age, Filipino, single and a resident of Poblacion Ilaya,
    Calinog, Iloilo, Philippines, after having been duly sworn to in accordance with law, hereby
    depose and state:

    1.) That I am an owner/holder of St.Peter Life Memorial Homes Official Receipt with
    O.R. No. 0004532 for the payment of excess kilometer and flexy glass for funeral
    services of my mother, Gloria L. Caro, who died on January 28, 2021 at Medicus
    Hospital, Iloilo City, Philippines;

    2.) That on February 05, 2021 at around 7am in the morning, I misplaced said Official
    Receipt of St.Peter Life at my house in Calinog, Iloilo, Philippines;

    3.) That despite serious efforts to locate the said Official Receipt, it cannot be found;

    4.) That I lost all hope in finding such Official Receipt, hence, I execute this Affidavit to
    establish the fact of loss of the receipt and for whatever legal purpose it may serve.
