In addition, NADOPOD is one of an organization’s Safety and Health Officer program for
detailed reporting any accident or incident, poisoning and workplace illness to the JKKP or
DOSH thoroughly. The objectives of NADOPOD are to comply with the act, to systematize
reporting and to verify a valid and dedicated statistics. Next, the form that already provided by
the DOSH must be used by the person who is reporting. The people who is responsible for
reporting an accident should use the standard form by the DOSH. Each form has their own
purposes. Therefore, the person who wants to report the incident must know the suitable and
correct form (Department of Safety and Health Malaysia (2004).
Besides, notification of accident and dangerous occurrence (Regulation 5 (1)) state that any
accident that occur or related with work that cause death, serious body injury (schedule 1) or
when dangerous occurrence (schedule 2) occurs must be reported by the employer to DOSH
and in within 7 days, the report must be submitted in an acceptable form which is JKKP 6. After
that, reporting of occupational poisoning and disease (Regulation 7). Firstly, the report must be
sent to the DOSH by an employer which a person at workplace who suffer from one of the
occupational disease or occupational poisoning in the 3rd Schedule form JKKP 7 within 7 days.
For the maintenance of records (Regulation 10), the employers must maintain records of all
accidents, dangerous occurrences, occupational diseases and occupational poisonings that
occurred that are related with work by using the approved form JKKP 8. For form JKKP 9 and
JKKP 10, it only used if the Director General of DOSH or any DOSH officer wants an extra data
for an incident or accident only. Accordingly, under the regulation 13, for the penalty, if either
party violates this regulation, the conviction must be liable to a fine of not more than RM10 000
or one year imprisonment or both as it agreed by Ammar (2017) and Department of Safety and
Health Malaysia (2004).
However, for this practical report, it is necessary to explain the accident report to DOSH for EXi
HOLDING SDN BHD company by using JKKP 6, JKKP 7, JKKP 8, JKKP 9 and JKKP 10
