    Parental / Guardian Consent Form

    This form should be filled by the parent(s)/legal guardian of the applicant, signed, scanned and submitted
    in the Attachment section of the online application.

    Omission of pertinent information important to acceptance of an applicant may result in the


    cancellation of the acceptance.

    It is understood that this signed application does not oblige the school to accept the student;
    however, once the Selection Panel has accepted the student and you have received an official
    acceptance letter from the New School, a contract is deemed to exist between the school and the
    parents/guardian, providing that the terms outlined in the school prospectus and the school
    regulations are observed.

    As parent / Legal guardian of the applicant, I certify that:

    The information on this application is complete, true, and accurate and that all relevant
    educational information about my child/ the applicant under my custody has been reflected in the
    application.

    I give my child/ the applicant under my custody permission to participate in the Eastern
    Partnership European School Scholarship for Secondary Education, for enrolment in the IB
    Programme with a Focus on European Studies at the New School, Tbilisi, Georgia.

    I agree to obtain a passport from the country of citizenship of my child/ the applicant under my
    custody and other required documents, including visas in case he/she is selected on the basis of
    academic screening, entrance exams and interview and meets the other Programme eligibility
    requirements.

    I understand that I may visit my child/ the applicant under my custody following the Boarding
    School policy, arranging the visit in a way that is convenient for the educational process. The
    expenses of parental meetings are not covered by the Scholarship.

    I understand that the entire application process for the scholarship is free of charge. However, the
    travel charges to the location where the testing will be organised have to be covered by the
    applicant’s family or guardian and are not reimbursable.
    I give my child/ the applicant under my custody permission to travel to and from Georgia and go
    on trips related to the IB DP studies, understanding that arrangements will be done in accordance
    to New School Student/Parent Handbook.

    I accept the services provided by the medical insurance company selected by the school.

    I accept the counseling and medical services provided by the school.

    I understand that my child may reside in the boarding school of the New School in Tbilisi, Georgia,
    under the school supervision, if boarding is requested as part of the online application of my child/
    applicant under my custody.

    I understand, that my child may be dismissed from the Diploma Program for behavior and
    academic issues that New School considers inappropriate or detrimental (as is defined in the
    Student/Parent Handbook).
